摘要
目的:分析门诊特殊患者的用药咨询,提高我院药学服务的质量。方法:调查2009～2011年门诊1 021例患者的药物咨询记录,针对其中307例特殊人群用药咨询进行回顾性分析并归纳解决的方案。结果:关于药物的成分、用法用量以及其不良反应等问题,在特殊人群用药咨询中比例较高,而在儿童和老年患者中,抗菌药和糖皮质激素类药物的合理应用也是主要考虑的问题。结论:通过药师与临床医师、护士相互沟通,以及自我知识的不断更新与积累,逐步形成科学、完善、便利的药物咨询系统,咨询与解决问题的良性循环,使患者获得最大的利益。
Objective： To analyze the drug consultation of special patients to improve the service quality of our hospital.Method： The drug consultation record of 1021 patients from 2009 to 2011 was investigated,and the consulting information of 307 special patients was retrospectively analyzed and the solutions were summarized.Result： The consulting proportion of the drug components,usage,dosage and adverse reactions was much high,and the rational use of antibiotics and glucocorticoids was the first consideration in the younger and older patients.Conclusion： The scientific,perfect and convenient drug consultation system should be established step by step on the basis of the communication among clinical pharmacist,clinician and nurse,accumulation and improvement of self-knowledge.The virtuous circle between consultation and solutions can ensure the greatest advantage for the patients.
