摘要
目的:了解戒烟后不同时期新诊断2型糖尿病合并下肢动脉硬化性疾病(LEAD)患者高敏CRP(hs-CRP)、纤维蛋白原及股动脉内膜中层厚度(IMT)的变化。方法:将120例初次明确诊断的2型糖尿病合并LEAD伴有吸烟史的男性患者分为吸烟组及戒烟组,每组60例。分别在戒烟前、3个月及1年后对hs-CRP、纤维蛋白原及股动脉IMT进行组间及组内自身对照比较。结果:戒烟3个月后hs-CRP、纤维蛋白原开始变化,1年后较戒烟前及吸烟组有明显改善(P均<0 05)。戒烟3个月后戒烟组与吸烟组的股动脉IMT无明显差异(0.89±0.21对比0.91±0.16),但12个月后戒烟组股动脉IMT与戒烟前及吸烟组比较差异具统计学意义(0.82±0.13对比0.89±0.17,P<0 05)。结论:戒烟对新诊断2型糖尿病合并LEAD患者hs-CRP及血浆纤溶活性呈持续改善作用,较短时间戒烟不能改善股动脉IMT,但戒烟1年后股动脉IMT有逆转性改变。
Objective: To evaluate the effects of smoking cessation on the expression of high sensitivity C-reactive Protein (hs-CRP) and fibrinogen (FIB), and the femoral artery intima-media thickness (IMT) of patients with newly diagnosed type 2 diabetes mellitus (T2DM) complicated with lower extremity arterial diseases(LEAD). Methods: One hundred and twenty patients with newly diagnosed T2DM combined with LEAD were randomized into 2 groups: smoking group and smoking cessation group. FIB, hs-CRP, and femoral artery IMT were assessed before, three months after, and twelve months after smoking cessation. Results: There were differences in Hs-CRP and FIB 3 months after cessation between the 2 groups, though the significant differences were first revealed 1 year after cessation (P 〈0. 05). Although there was no significant improvement of IMT in cessation subjects at the 3rd month, after 1 year, there was significant improvement in IMT in cessation group (0. 82±0. 13 vs 0. 89 ±0. 17, P 〈 0. 05 ). Conclusion: Smoking cessation results in improvement in Hs-CRP and FIB in patients with newly diag- nosed T2DM complicated with LEAD. Although the short time effect on the IMT is not confirmed, a 1-year smoking cessation can significantly reverse the thickened IMT.
