三维增强磁共振血管成像在下肢动脉硬化病变中临床应用

摘要 目的探讨三维增强磁共振血管成像(MRA)对下肢动脉硬化病变检查的临床应用价值。方法对24例下肢动脉硬化病变的患者,进行三维增强MRA检查,用3DSPGR序列采集注射造影剂之前和之后的图像,进行数字减影处理,然后进行最大信号投影重建。图像质量分为优、良、差3级,同时观察下肢动脉狭窄、闭塞的情况。结果本组24例病人中,质量优18例,良5例,差1例。24例中发现阳性者23例,其中多发动脉斑块无明显动脉管腔狭窄者1例,多发动脉斑块形成伴管腔狭窄15例,多发斑块伴动脉部分节段闭塞7例。18例动脉管腔重度狭窄或闭塞的病例显示了侧支循环血管。结论三维增强MRA可有效显示下肢动脉血管的异常,在下肢动脉病变检查中具有重要的临床意义。 Objective To assess the clinical application of three-dimensional contrast- enhanced MR angiography ( 3D CE-MRA) in the detection of the lower extremity arteriosclerosis disease. Methods 3D CE- MRA were performed in 24 patients with lower extremity arteriosclerosis disease. Three dimensional spoiled GRE was used to acquire imaging, the pre-contrast mask sequence was then subtracted from the post-contrast sequence. Maximum intensity projections were calculated from the subtracted data sets. The image quality was evaluated by three different levels (good, fair, and poor), and the demonstration of various stenotic and/or occlusive in lower extremity were observed. Results In all 24 patients, we determined that 18 are good quality images, 5 are fair quality images and 1 is poor quality images. Normal arteries were showed in 1 cases, plaques of atherosclerosis without stenoses 1, with stenoses 15, with occlusions 7. Collateral circulation were showed in 18 cases with stenoses or occlusions. Conclusion 3D CE-MRA can be able to clearly show the lower extremity vascular abnormality and has important significance in diagnosis of Low limb artery disease.
