摘要
目的:通过对2010年1月—2011年12月病历医嘱用药情况进行分析,了解住院患者用药情况,对不合理用药情况进行分析归类,以促进临床医师合理安全用药。方法:采用回顾性分析方法,结合患者病情和用药情况对抽查病历中的医嘱用药进行综合审查、分析。结果:随机抽取住院5 d以上、非死亡患者病历960份,其中存在不合理用药医嘱247份,占总抽查病历的25.73%,主要表现在溶剂选择不适当、配伍不适宜、用法与用量不正确、重复给药、选药不合理等。结论:医师应重视对药品说明书和药物配伍知识的了解,药师应加大药学服务力度,为临床医师合理用药提供有力支持和帮助,相互配合减少不良反应(ADR)的发生,保证用药安全及临床治疗效果。
OBJECTIVE ： To probe into the drug use of the inpatients through analyzing the medical orders on drug use in our hospital from Jan. 2010 to Dec. 2011 and to analyze and categorize the irrational administration of drugs so as to promote clinical rational and safe medication. METHODS： By a retrospective study, the drug use was reviewed and analyzed comprehensively by combining patients＇ state of illness. RESULTS ： A total of 960 medical records of non-death cases （hospitalized for over 5 days） were randomly sampled, of which, 247 （25.73%） were found to have irrational use of drugs, which manifested as irrational choice of solvents, incompatibility, improper dosage and administration, irrational choice of drugs etc. CONCLUSION： Physicians should attach great importance to drug package inserts and drug compatibility knowledge, meanwhile, pharmacists should strengthen pharmaceutical care and provide strong support for clinicians in rational drug use so as to reduce the frequency of adverse drug reactions and ensure medication safety and efficacy of clinical treatment.
