摘要
目的:观察短期降糖治疗对冠心病并糖尿病(diabetes mellitus,DM)患者臂-踝脉搏波速度(brachio-ankle pulse wave velocity,baPWV)的影响。方法:选择冠心病并DM患者70例,在冠心病规范化治疗、生活方式干预的前提下,服用二甲双胍片和格列齐特缓释片治疗6个月,观察治疗前后血糖、血脂、糖化血红蛋白(glycosylated hemoglobin,HbA1c)、超敏C-反应蛋白(high sensitive C-reactive protein,hs-CRP)、一氧化氮(nitric oxide,NO)、内皮素(endothelin,ET)及baPWV的变化。结果:治疗后甘油三酯、低密度脂蛋白胆固醇、空腹血糖、餐后2 h血糖、HbA1c及hs-CRP较治疗前显著下降(P<0.01),NO水平显著上升(P<0.01),ET水平及baPWV较治疗前相比无显著差异(P>0.05)。结论:冠心病并DM患者在冠心病规范化治疗、生活方式干预的前提下,短期服用二甲双胍片和格列齐特缓释片并不能逆转患者的PWV而改善动脉弹性。
Objective: To investigate the effects of brachio-ankle pulse wave velocity(baPWV) in patients with coronary artery disease and diabetes mellitus(DM) after short term antidiabetic therapy.Methods: A total of 70 DM patients with coronary artery disease were selected.Under the premise of standardized treatment and lifestyle intervention to coronary heart disease,taking metformin and gliclazide treatment for 6 months,the changes of blood glucose,blood lipid,glycosylated hemoglobin(HbA1c),high sensitive C-reactive protein(hs-CRP),nitric oxide(NO),endothelin(ET) and baPWV before and after treatment were observed.Results: Levels of teriglyceride(TG),lowd ensity lipoprotein-cholesterol(LDL-C),fasting plasma glucose(FPG),2 hour postprandial blood glucose(2 h PG),HbA1c and hs-CRP are significantly decreased after than before treatments.(P0.01),level of NO significantly increased(P0.01),levels of ET and baPWV have no significant differences(P0.05).Conclusion: Under the premise of standard treatment and lifestyle intervention,metformin and gliclazide treatment can not reverse PWV of DM patients with coronary artery disease after short therapy,improve elasticity of aorta.
