摘要
目的探讨食管结核的手术方式及手术适应证。方法回顾性分析国内1998年1月1日～2011年9月30日所有文献报道过经手术治疗的食管结核病例107例以及笔者医院手术1例患者的诊治经过。本组病例共108例,其中行病变食管切除+胃食管吻合术73例,局部病灶清除术27例,开胸探查后关胸7例,食管狭窄环松解术2例。所有病例手术后都经过抗结核药物治疗6～18个月。结果本组病例全部治愈,术后随访3个月～20年均无复发。结论对于在术中确诊的食管结核患者,术前吞咽困难在1～2级者只需行局部坏死组织清除术,如吞咽困难在3～4级的,可加做食管狭窄环松解术。术后辅以抗结核治疗12个月,同样能治愈。
Abstract Objective To explore the surgical procedures and the indications for esophageal tuberculosis. Methods The surgical treatment was retrospectively reviewed in 108 cases which all literature retrieved from Wanfang Database from 1 January 1998 to 30 September 2011, and one was operated in our hospital. Among 108 patients, 73 patients were operated with esophagectomy and esophagogastr- ostomy, 27 patients underwent the local of lesions clean, 7 patients had no operation after exploratory thoracotomy, 2 patients underwent the local of lesions clean and decompressed in tensive position of esophagus. All of the postoperative patients has been implemented antitu- bercular treatment for 6 to 12 months. Results All of the patients were cured, and no patients relapsed daring the follow - up visits from 3 months to 10 years. Conclusion If the patients of esophageal tuberculosis who are identified by exploration in operation are in class 3 - 4 of dysphagia,They may unclergo the local of lesions clean and decompressed in tensive position of esophagus. And the patients with class 1 - 2 need be only put the local of lesions clean. Antituberculosis treatment was taken for each of the patients after operation. The petients were healed, too.
