摘要
目的:介绍一种新的保留包皮治疗成人先天性包茎的手术方法。方法:对2008年4月～2011年3月间117例门诊患者应用四Y-V成形术治疗,恢复其包皮上翻性,术后随访12个月,观察患者包皮外观形态以及并发症情况。结果:117名患者术后均能够将包皮顺利上翻并获得了满意的外观,5～14天消肿(平均8.0天),7～18天伤口愈合(平均11天)。早期18例患者发生轻度瘢痕挛缩(15.4%),其中13例半年后自行缓解,5例局部外用激素治疗后治愈。结论:四Y-V成形术可以恢复成人先天性包茎患者正常的包皮功能和外观,避免了传统包皮环切术切除包皮的弊端,治疗效果好。
Objective To introduce a new surgical procedure for adult phimosis. Methods 117 outpatient cases of adult phimosis were treated with four Y-V plasties, in April 2008 to March 2011. 12 month postoperative follow up was made to observe the retract ability, cosmetic results and complications. Results All patients could retract their prepuces easily and acquired satisfactory appearances. The mean time of detumescence and wound healing were 8.0 and 10.7 days. There was mild scar contracture in 18 cases （15.4%）which were cured by topical steroid application. Conclusions Four Y-V plasties is an excellent prepuce saving therapy for adult phimosis that provides near-normal prepuce appearance and function.
