摘要
目的探讨脑保护装置(EPD)在颈动脉狭窄支架成形术中的应用价值。方法回顾性分析2004年4月～2010年10月采用脑保护装置对42位颈动脉狭窄患者47支靶血管施行颈动脉狭窄支架成形术,分析和总结手术成功率,术中、术后并发症,以及并发症的预防和处理等。结果手术成功率100%,狭窄改善程度由(76.14±12.86)%下降到(16.45±8.37)%,术前术后对比有显著差异(P≤0.01)。并发症发生率7.14%,其中术中1例出现同侧大脑前动脉闭塞,2例出现微栓子脱落导致轻度脑梗塞,对症治疗后恢复,6例(12.8%)脑保护装置中可见斑块碎片。结论颈动脉支架+脑保护装置是治疗颈动脉狭窄的有效手段,能明显降低术后缺血性脑卒中的发生率。
Objective To investigate the value of cerebral protection device in carotid angioplasty and stenting.Methods 42 patients with carotid artery stenosis were treated with carotid angioplasty and stenting under cerebral protection device.The success rate of surgery,intraoperative and postoperative complications were observed.Results All of them were completely successful and significantly effective.The mean degree of stenosis decreased from 76.14±12.86% to 16.45 ±8.37%.Complication rate was 7.14%.3 patients appeared symptomatic cerebral embolism in the intraoperative and postoperative.Plaque debris were found in 6 cerebral protection devices（12.8%）.Conclusion Carotid angioplasty and stenting with cerebral protection device are an effective theraputic method in the treatment of carotid stenoses.Cerebral protection device can significantly reduce the postoperative incidence of ischemic stroke.
