摘要
目的 探讨胃癌穿孔的临床特点 ,指导临床诊治和提高疗效。方法 对本院 2 7例胃癌穿孔病人采用不同手术方式治疗。其中 5例行单纯穿孔修补术 ,2例在穿孔修补术后 3周早期施行二期根治性手术 ,3例行穿孔修补和胃空肠吻合术 ,12例行姑息性胃大部切除术 ,4例行胃癌根治术 ,1例行胃造口。结果 本组死亡 5例。经手术者平均生存期分别为 8个月、13个月和 2 7 2个月。结论 早期诊断 ,综合治疗 ,是降低病死率提高生存质量的重要因素。
Objective To investigate the clinical characteristics and give better guide to the diagnosis,therapy,and improvement of prognosis for the patients suffering from perforation of gastric cancer.Methods We summarized 27 cases suffering from perforation of gastric cancer in our hospital from 1986 to 1999.Of the patients,5 were treated with repair of perforation,2 with radical gastrectomy for gastric cancer after repair of perforation about three weeks,3 with repair of perforation and gastrojejunostomy at the same time,12 with partial gastrectomy,4 with radical gastrectomy for gastric cancer,1 with gastrostomy.Results Five patients died during the perioperative period.The life expectancy of patients treated with repair of perforation,partial gastrectomy or radical gastrectomy for gastric cancer was 8,13 and 27 2 months,respectively.Conclusion Early diagnosis and great attention to whole treatment are important factors to cut down the death rate and increase life quality.
