七氟醚吸入用于面神经监控下腮腺及面神经探查手术麻醉的研究 被引量：2

摘要 目的观察不同浓度下的七氟醚吸入对腮腺及面神经探查手术中的面神经实时监控下面神经肌电活动收集的影响。方法 20例择期全麻下行腮腺及面神经探查手术患者,气管插管后采用七氟醚吸入麻醉,调整七氟醚呼出入浓度为0.4、0.6、0.8、1.0、1.2、1.4、1.6 MAC时,观察血压、心率及面神经肌电活动波形的振幅变化(振幅大于100 mV监测有效),同时记录BIS值的变化。结果神经监护仪所收集的面神经电活动振幅随着七氟醚呼出浓度的逐渐增高,而使其逐渐降低,但振幅均大于130 mV,全组患者的面神经实时监控均未中断;当七氟醚呼气末浓度高于1.6 MAC时,BIS值低至30左右,神经肌电活动监测亦未中断。当七氟醚呼浓度低于(0.1±0.1)MAC,BIS值在90～94之间时,全组患者清醒。全组患者血流动力学指标与麻醉前相比均轻度下降(P>0.05),有显著差异。结论静脉快速诱导、单纯七氟醚吸入全麻,既能维持腮腺及面神经探查手术的血流动力学平稳,又不会影响面神经功能的监控,而且苏醒迅速、完全,神经监控手术中可以推荐使用。
