脑挫伤早期神经细胞凋亡和Caspase-3表达变化的观察 被引量:1

Changes of Neuronal Apoptosis and Expression of Caspase-3 after Human Brain Contusion at Earlier Stage

摘要 目的观察脑挫伤后早期神经细胞凋亡和调节因子Caspase-3表达的时序及空间变化规律。方法采用TUNEL染色和免疫组化染色方法,结合图像分析技术,对不同损伤时间组人脑挫伤组织进行染色观察。结果①TUNEL染色和Caspase-3免疫组化染色的阳性细胞主要分布在挫伤区和挫伤半影区,与远隔部位和对照组脑组织的阴性染色有显著差异,P<0.05;②脑挫伤后48 h内,在挫伤半影区TUNEL染色的阳性率和阳性细胞IOD逐渐增加,各时间组之间有显著差异,P<0.05,两参数与损伤时间均呈线性关系,r分别为0.89和0.93;③Caspase-3阳性表达与神经细胞凋亡变化呈平行关系,两参数与损伤时间也呈线性关系,r分别为0.92和0.90。结论①人脑挫伤后48 h内神经细胞凋亡和Caspase-3阳性表达的阳性率、IOD增加与损伤时间之间呈线性关系,可作为损伤时间推断的新依据;②TUNEL和Caspase-3免疫组化染色阳性细胞主要分布在挫伤半影区和挫伤区都提示可以用于判断脑挫伤部位,尤其是对轻度脑损伤大体和HE染色形态改变不明显的脑挫伤更有意义。 Objective To observe the temporal and spatial changes of neuronal apoptosis and expression of caspase-3 after human brain contusion at earlier stage. Methods The slides of the contusive tissue of human brain were stained by TUNEL, Caspase-3 immunohistochemistry methods. Image analysis technique was employed. Results ① The positive cells of TUNEL and Caspase-3 immunohistochemistry were distributed in the contusive area and the penumbra area of contusion. The positive staining was striking contrast with distal area or control groups, P 〈 0.05. ②The rate of positive cells and the IOD of TUNEL increased linearly in penumbra area of contusion in 48 hours after brain injury. There were significances different between groups,P 〈 0.05. Furthermore,There were linear relationship between two parameters and the timing of injury. R was 0.89 and 0.93. ③The changes of expression of Caspase-3 were parallel to the change of neuronal apoptosis. There was linear relationship too between two parameters and the timing of injury. R was 0.92 and 0.90. Conclusion ①The linearly increase of neuronal apoptosis and expression of Caspase-3 are useful in timing of brain contusion. ②The distribution of positive cells of TUNEL and Caspase-3 immunohistochemistry staining hint to locating the contusion in brain,especially it is not discovered by gross examination and H. E staining in the mild brain injury.
