孕激素及左炔诺孕酮宫内缓释系统治疗子宫内膜增生症疗效观察被引量：43

摘要目的观察孕激素及左炔诺孕酮宫内缓释系统(levonorgestrel-releasing intrauterine system,LNG-IUS)治疗子宫内膜增生症的临床疗效。方法 50例无排卵性功能失调性子宫出血子宫内膜增生症患者随机分为2组,孕激素组30例于诊断性刮宫术后第5天给予醋酸甲羟孕酮8mg/次,1次/d,连续口服22d,然后于下次月经第5天继续口服醋酸甲羟孕酮(用法及用量不变),共服用3个月经周期;LNG-IUS组20例于诊断性刮宫术后第5天宫腔内置入LNG-IUS。比较2组治疗前、后月经量改变情况、血红蛋白水平、子宫内膜厚度、子宫内膜逆转率及不良反应发生情况。结果孕激素组治疗第1个月月经过多24例,LNG-IUS组无月经过多患者,2组比较差异有统计学意义(P<0.05);LNG-IUS组治疗第1,2个月血红蛋白水平高于孕激素组(P<0.05),治疗第3个月子宫内膜厚度低于孕激素组(P<0.05),子宫内膜逆转率高于孕激素组(P<0.05);孕激素组主要不良反应为体质量增加,恶心、呕吐,肝、肾功能损害,LNG-IUS组主要不良反应为不规律点滴状子宫出血。结论宫腔内置入LNG-IUS治疗子宫内膜增生症效果优于口服孕激素,且不良反应轻。
