ESWAN序列在脑微出血诊断中的价值研究 被引量:7

The value of ESWAN in diagnosis of cerebral microbleeds

摘要 目的:采用多回波采集重度T2*WI三维梯度回波序列(ESWAN)研究脑卒中患者与正常人群中脑微出血灶(CMBs)的发生率与分布的差异及ESWAN在诊断脑微出血中的优势。方法:采用ESWAN检测30例脑梗死、30例脑出血和30例健康老年人的CMBs,按照CMBs数目将其分为无、轻、中和重度4级,对不同组之间CMBs严重程度的差异以及ESWAN与T2*GRE序列、常规MRI序列(T1WI、T2WI、FLAIR)对微出血灶检出敏感性进行统计学分析。结果:脑出血组的CMBs发生率为76.6%,以中重度为主;脑梗死组为36.7%,以轻中度为主;正常老年人为6.7%。各组间CMBs差异具有统计学意义(P<0.05)且CMBs最常见于大脑半球深部区域,其次是脑叶皮层和皮层下区。ESWAN序列对病灶的检出率明显高于T2*GRE及常规序列。结论:CMBs在脑卒中患者中发生率较高,ESWAN序列在脑微出血的诊断中与T2*GRE及常规序列相比具有明显优势。
