摘要
目的比较电凝联合内镜下注射血凝酶与钛夹联合内镜下注射血凝酶治疗重症消化性溃疡出血的疗效及安全性。方法 82例消化性溃疡出血患者随机分成两组,一组42例电凝止血后予内镜下注射血凝酶治疗;另一组40例钛夹钳夹止血后予内镜下注射血凝酶治疗。结果两组患者镜下治疗后即时止血率均为100%,止血成功率分别为80.5%和87.5%,两组对比差异无统计学意义。对于Forrest分级Ⅰa、Ⅱa出血灶,电凝组与钛夹组止血成功率分别为61.9%(13/21)与95.0%(19/20),钛夹组止血成功率高于电凝组。结论电凝联合内镜下注射血凝酶与钛夹联合内镜下注射血凝酶治疗消化性溃疡出血起效迅速,疗效确切,并发症少。但两者各有优缺点,可根据不同情况灵活选用。
Objective To evaluate the effect and safety of electrocauterization and metal titanium clips combined with in- jection hemocoagulase under endoscopy to peptic ulcer bleeding. Methods Totally 82 patients with peptic ulcer bleed- ing were randomly divided into two groups. 42 cases of patients treated with electrocauterization combined with injection hemoeoagulase, 40 cases of patients treated with metal titanium clips combined with injection hemocoagulase., Results Initial haemostatic rate was 100% in both groups. The haemostatic rate was 80. 5% and 87, 5% in electrocauterization group and metal titanium clips group respectively, there were no significant differences between the two groups （P 〉 0. 05）. There were prominent statistical differences between Forrest grade I a and II a in two groups （ P 〈 0. 05 ）. Conclusions The endoscopic eleetroeauterization and metal titanium clips combined with injection hemocoagulase on the treatment of peptic ulcer bleeding also is safe and effective.
