脊柱结核植入内固定安全性的体外实验研究 被引量:5

摘要 目的通过体外实验研究观察结核杆菌和不同内植物材料的黏附情况,并与表皮葡萄球菌进行对照,探讨脊柱结核行病灶清除后植入内固定的安全性问题。方法以表皮葡萄球菌为对照,将临床常见的不同内植物材料(不锈钢,钛,钛合金)置入结核杆菌、表皮葡萄球菌的菌液,行细菌培养后通过扫描电镜观察比较不同细菌对不同材料的黏附情况,统计分析结果。结果结核杆菌相对于同一种材料的黏附数量均较表皮葡萄球菌少(P<0.01),而在不同材料的结核杆菌黏附比较中,钛及钛合金的细菌黏附数量少于不锈钢(P<0.01)。结论在脊柱结核患者体内植入内固定是相对安全的,内固定材料选用钛或钛合金优于不锈钢。 Objective To study the adhesion of Mycobacterium tuberculosis onto the surface of different implant materials in vitro,and to evaluate the safety of internal fixation implants in spinal tuberculosis through comparison with Staphylococcus epidermidis.Methods Staphylococcus epidermidis was selected as the control.Common implant materials(stainless steel,titanium and titanium alloy) were individually placed in the different bacterial suspensions and the adherence of Mycobacterium tuberculosis and Staphylococcus epidermidis onto the different implant materials was observed by scanning electron microscopy(SEM)after bacterial culture.Results The adherence of Mycobacterium tuberculosis onto the surface of each material significantly decreased compared with Staphylococcus epidermidis(P〈0.01).The number of Mycobacterium tuberculosis adhered onto the surface of pure titanium or titanium alloy significantly decreased compared with stainless steel(P〈0.01).Conclusion Instrumentation surgery is relatively safe in patients with spinal tuberculosis.As the implant materials,titanium alloy and pure titanium are superior to stainless steel for internal fixation.
