摘要
目地探讨地塞米松不同的用量及注射方式对降低CT增强扫描时注射非离子型碘对比剂引起的不良反应的作用。方法对1996年6月—2011年7月共1558例进行增强扫描的患者进行回顾性分析,按照地塞米松使用方法不同分为3组。药物I组为10mg地塞米松与造影剂混合一起高压注射器团注,共164例。药物II组为注射造影剂前15min静脉推注10mg地塞米松,共622例。药物III组为注射造影剂15min静脉推注5mg地塞米松,共772例。观察注射造影剂时及后30min的不良反应情况并记录。结果药物I组不良反应发生率为62.8%,其中,轻度不良反应占不良反应总数的96.12%,中、重度不良反应占不良反应总数的3.88%,药物II组不良反应发生率为28.78%,其中,轻度不良反应占不良反应总数的99.44%,中、重度不良反应占不良反应总数的0.56%,药物III组不良反应发生率为29.53%,其中,轻度不良反应占不良反应总数的99.56%,中、重度不良反应占不良反应总数的0.44%。药物I组轻度不良反应发生率明显高于药物II和药物III组(P<0.05),药物II组与药物III组轻度和中重度不良反应发生率差异无统计学意义(P>0.05)。结论注射非离子型碘对比剂前15min静脉推注地塞米松能显著降低轻度及中度不良反应发生率,用量以5mg为宜。
Objective In order to explore the role of distinct dosages and manners ofinjection of dexamethasone played in reducing side reactions cause by non-ionic iodine Contrast Agent injection in enhanced CT scan. Comparative Methods A total of 1558 cases of enhanced scan patients were analyzed retrospectively from June 1996 to July 2011, and were divided into 3 groups according to the applicative manner and dosage of dexamethasone. In group Ⅰ, 10 mg dexamethasone was injected along with iohexol by high-pressure syringe, 164 patients in all. In group Ⅱ, 10 mg dexamethasone was injected 15 minute before injecting iohexol, and there were 622 patients in all. In group Ⅲ, 5 mg dexamethasone was injected 15 minute before injecting iohexol, and there were 772 patients in all. And to observe and record the side reaction of during enhanced CT scan and within 30 minutes of the scan. Results The incidence rate of side reaction of group Ⅰ were 62.8%, including 96.12% slight side reactions and 3.88% medium or serious side reactions. The incidence rate of side reaction of group Ⅱ were 28.78%, including 99.44% slight side reactions and 0.56% medium or serious side reactions. The incidence rate of side reaction of group Ⅲ were 29.53%, including 99.56% slight side reactions and 0.44% medium or serious side reactions. The incidence rate of slight side reaction in group Ⅰ were significantly higher than group Ⅱ and group Ⅲ(P0.05).Conclusion Injection of 5 mg dexamethasone 15 minutes before the injection of non-ionic iodine contrast agent could reduce the incidence of side reaction obviously cause by the contrast agent.
