腹主动脉缩窄兔血清脑钠肽水平与超声心功能指标的相关性研究

Study on the Correlation Between Serum Brain Natriuretic Peptide and Heart Function Indicesby Ultrasonography in Abdominal Aortic Coarctation of Rabbits

摘要 【目的】探讨腹主动脉缩窄兔血清脑钠肽(BNP)水平与超声心功能指标的相关性。【方法】新西兰兔25只建立心肌肥厚一心衰动物模型,随机分为手术组与对照组,手术组15只用3号丝线于肾动脉上方约1crn将腹主动脉及导管一起结扎,缩窄腹主动脉内径60%左右,对照组10只仅在游离的腹主动脉处穿过丝线,未结扎。检测两组BNP水平,分析其与超声指标:左房内径(LA)、室间隔厚度(IVS)、左室后壁厚度(LVPw)、左室舒张末内径(LVDd)、左室射血分数(EF)、左室短轴缩短率(FS)的相关性。【结果】与对照组比较,BNP水平于术后2周即显著性增高,随后呈进行性增高(P〈0.05);LA、LVPW、IVS术后4周均显著增大,6周后LVDd明显增大,EF及FS术后8周明显下降(P〈0.05);BNP水平与LA、IVS、LVPW、LVDd呈正相关,与EF、FS呈负相关。【结论]BNP水平能够在超声指标出现异常之前早期发现心功能异常并可提示心功能损害程度,且与超声心功能指标具有良好的相关性。 [Objective] To explore the correlation between serum brain natriuretic peptide(BNP) and heart function indices by ultrasonography in abdominal aortic coarctation of rabbits. [Methods] The models of ven- tricular hypertrophy and heart failure in 25 New Zealand rabbits were established. All the rabbit models were randomly divided into operation group and control group. The operation group( n = 15) underwent the ligation of both abdominal aorta and catheter at about lml above renal artery by using 3rd thread, and the inner diame- ter of constrictive abdominal aorta was about 600/00. The control group( n = 10) only underwent the thread passing of free abdominal aorta without ligation. BNP level of two groups was measured. The correlation of left atrial diameter(LA), interventrieular septal thickness (IVS), left ventrieular posterior wall thickness (LVPW), left ventricular end diastolic dimension(LVDd), left ventricular ejection fraction(EF), left ventric- ular minor axis decurtation rate(FS) with BNP was analyzed. [Results] Compared with control group, BNP level significantly increased 2 weeks after operation, and subsequently increased progressively( P ~ 0.05). LA, LVPW and IVS significantly increased 4 weeks after operation. LVDd obviously increased 6 weeks after operation. EF and FS obviously declined 8 weeks after operation( P 〈0.05). BNP level was positively corre- lated with LA, IVS, LVPW and LVDd, and negatively correlated with EF and FS. [Conclusion]BNP level can early detect the abnormal heart function before the appearance of abnormal ultrasonography index, which indicates the degree of cardiac dysfunction. BNP level has good correlation with heart function indices.
