摘要
目的探讨肾错构瘤自发破裂出血的临床诊断和治疗方法。方法回顾性分析我院16例肾错构瘤自发破裂出血患者的临床资料,所有患者均因腰腹部疼痛就诊,均行B超和CT检查。10例患者行患肾切除术,4例行肿瘤剜除术,2例行选择性肾动脉栓塞术。结果 14例手术患者术后恢复良好,随访8～48个月,未见肿瘤复发。2例行选择性肾动脉栓塞的患者,随访15和18个月,肿瘤明显缩小,未见再出血。结论 B超和CT是确诊肾错构瘤破裂出血简单而有效的方法。对于直径>4cm的肾错构瘤,应积极行手术治疗,并尽量保留有功能的肾组织。
Objective To improve the diagnosis and treatment of spontaneous rupture of renal hamartoma. Methods Sixteen cases of renal hamartoma rupture were reviewed. All the cases were diagnosed by ultrasound and spiral CT. 10 cases under- went nephrectomy,4 cases received nephron sparing surgery and 2 cases received selected transcatheter arterial embolization. Re- suits All the cases were recovered well after operation. All of them had no evidence of recurrence by followed up from 8 months to 48 months. Conclusion Ultrasound and CT scan are the preferred examination. The main therapy is nephrectomy , and some- times nephron-sparing surgery can be performed.
