射频消融对离体猪肺消融靶区内不同部位灭活效果的实验研究 被引量:3

摘要 目的探讨射频消融靶区内不同区域灭活效果的差异。方法 36副新鲜离体猪肺分为6个消融时间组进行射频消融,消融过程中记录消融中心区、射频针的中心针针尖区、边缘针针尖区、中心针与边缘针针尖连线的中间区的温度变化,消融结束后在4个消融区域及对照区采集标本,分别HE染色和Annexin V-FITC/PI标记(流式细胞术),计算在不同消融时间作用下不同区域的校正细胞死亡率。结果消融区域的校正细胞死亡率高于对照区域(P<0.05);在完全消融前,消融中心区的校正细胞死亡率高于3个消融边缘区(P<0.05);在短时间消融组(5~15 min),射频针的中心针针尖区与边缘针针尖区的校正细胞死亡率均高于中心针与边缘针针尖连线的中间区(P<0.05);而在消融时间达到20 min后,3个边缘消融区消融效果的差异无统计学意义;在完全消融前,各消融区域的校正细胞死亡率会随着时间的延长而增加;当消融时间达到30 min时,4个消融区域消融效果的差异无统计学意义,均可达到完全消融。结论在完全消融前,消融中心区的消融效果要优于消融边缘区;消融边缘区之间在短时间消融时存在消融效果的差异,这种差异会在达到一定的消融时间后消失;给予足够的消融时间,消融靶区可达到完全消融。 Objective To analyze the effect of radiofrequency ablation in different zones of the target area. Methods Thirty-six pulmonary tissues of pig lungs were divided into six different groups according to different ablation time. During the ablation procedure, the temperature change situation of the zone of ablation center, the needle tip zone of the central needle of RF needle, the needle tip zone of edge needle, the middle zone on the straight line between central needle tip and the edge needle tip were recorded. After ablation, samples were collected from four different zones of the target ablation areas and the comparison zones, dyed by hematoxylin and eosin and stained by Annexin V-FITC/PI. Flow cytometry(FCM)was used to determine the apoptosis. The corrected cell death rate of different zones in different ablation time was calculated. Results The corrected cell death rate of ablation zone was higher than the one of comparison zone in every group ( P 〈 0. 05 ). Before the cells disappeared completely, the corrected cell death rate of the ablation center was higher than the one of the other three ablation edge zones (P 〈 0.05 ). In short time ablation group(5 min, 10 min, 15 min) ,the corrected cell death rates of the needle tip zone of RF needle ablation center and the needle tip zone of edge needle tip zone were higher than that of the middle zone on the straight line between Central needle tip and the edge needle tip(P 〈 0.05 ). When the ablation time achieved 20min, there was no difference in the effect of ablation among the three ablation edge zones. Before the cells disap- peared completely, the corrected cell death rate of ablation zones would increase along with time extension. When the ablation time achieved 30 min, the cells in four ablation zones may disappear completely. Conclusion Before the cells disappear completely, the ablation effect of the ablation center is better than that of ablation edge zones. In short time ablation, there are differences among different ablation edge zones, but these differences will disappear if the ablation time extends adequately. All target zones could disappear completely if given enough ablation time.
