摘要
目的高分子聚氨酯绷带强度远超传统熟石膏,但其可塑性不如传统石膏,因此在下肢骨性突起处易引起卡压痛。本研究采用高分子聚氨酯绷带结合熟石膏绷带内衬固定,来探讨解决这一问题的可行性。方法将306例下肢骨折患者(159例为石膏托固定,147例为石膏管型)随机分成三组,即单纯聚氨酯绷带固定,熟石膏绷带固定,以及聚氨酯绷带结合熟石膏绷带内衬固定。随访比较各组石膏断裂及石膏内卡压的发生率。结果单纯聚氨酯绷带固定组石膏内卡压的发生率明显高于其余各组;熟石膏绷带固定组石膏断裂发生率明显高于其余二组;聚氨酯绷带结合熟石膏绷带内衬固定治疗效果理想,石膏断裂及石膏卡压的发生率均较低(P〈0.01)。该组102名患者均按时拆除石膏康复,外固定效果良好。结论使用高分子聚氨酯绷带结合熟石膏绷带进行骨科下肢石膏托和长腿石膏管型固定可取得良好效果。
Objective Although the polyurethane resin has been widely used for immobilization of fracture, it was not suitable for management of lower extremity fractures, which is usually so hard and may induce uncomfortable of the affected limb. We combined it with traditional plaster to improve the outeome of fractures immobilization, methods 306 patients with lower limb fractures were assigned into 3 groups: A, polyurethane resin group; B, plaster fixation group; C, combined polyurethane resin and plaster group. In each group, the patients were treated with either splint or cast according to the type and timing of the fractures. Results Compared with group C, there was a higher incidence of collapse in patients of plaster immobilization group. And in polyurethane resin patients, 43% (44 / 102) complained local pain with pressure mark of skin. In contrast, only 2% (2/102) of the patients immobilized with combination of polyurethane resin and plaster bad the similar symptoms. The statistical analysis showed a significant difference (P〈0.O1). Conclusion The combined application of polyurethane and plaster may provide an effective method for immobilization of fractures of the lower limb and avoid the complication of polyurethane.
