摘要
目的探讨多西他赛加洛铂方案与适形放疗同步治疗中晚期食管癌的临床疗效及毒性反应。方法42例经组织学或细胞学证实的局部晚期食管癌随机分为两组，同步放化疗组21例接受同步放化疗，单纯放疗组21例仅接受放疗，化疗采用TP方案，放疗剂量为56－60Cy，治疗结束后分别观察疗效及毒副反应。结果观察组和对照组有效率分别为90．4％、665％，差异有统计学意义（P〈0．05）；同步放化疗组和单纯放疗组1、2、3年生存率分别为714％、47．6％、38．1％和52．4％、28．6％、14.3％（P〈0.05）；同步放化疗组和单纯放疗组毒副反应发生率差异无统计学意义（P〉0．05）．且均无严重毒副反应发生。结论同步放化疗治疗局部晚期食管癌疗效有优于单纯放疗的趋势，且不增加严重毒副反应。
Objective To evaluate the efficacy and tolerability of three dimensional cenfonnal radiotherapy in plus concurrent lobaplatin with docetaxel for locally advanced esophageal carcinoma Methods Forty-two patients with locally advanced esophageal cancer were divided into two groups.The twenty-one patients were treated with concurrent radiotherapy and chemotherapy;the other 21 patients were treated with radiotherapy alone.The chemotherapy regimen was TP regimen, the radiotherapy dosage was 56-60 Gy.The efficacy and toxieities were respectively observed after treatment. Results The overall response rate of radiotherapy arm and concurrent with docetaxel and lobaplatin arm was 90.4% mad 66.5% respectively.There was a significant difference between concurrent with docetaxel arm and radiotherapy only arm （P〈0.05）.There l-year,2-year, 3-year overall survival rate （OS） was 71.4% ,47.6% ,38.1%~11 52.4% ,28.5% ,14.3% respectively.The OS had a significant difference between all arms （P〈0.05）.There were no serious toxicities in the two groups;there was no significant difference in the incidence ratea of the two groups （P〉0.05）.Conclusion The efficacy of concurrent radiotherapy and chemotherapy seems to be better than that of the radiotherapy alone in the treatment of locally advanced esophageal cancer,and does not increase the serious toxicities.
