摘要
目的评估腹膜外结肠造口术后功能锻炼对增强排便信号的作用和腹部造口并发症的影响,探讨改善直肠癌患者术后生活质量的方法。方法〗将在本院进行手术治疗的51例直肠癌患者分为腹膜外结肠造口手术治疗组（对照组,n=23）和腹膜外结肠造口术后功能锻炼组（功能锻炼组,n=28）,对两组手术后排便信号和围术期腹部造口并发症进行分析和比较。结果对照组：排便前5~10 s出现排便信号2例（8.69%）、11-20 s 10例（43.48%）、21-30 s 7例（30.44%）、≥30 s 1例（4.34%）;围术期造口缺血3例,造口黏膜皮肤分离2例,造口周围炎8例,造口回缩2例。功能锻炼组：排便前5~10 s出现排便信号2例（7.14%）、11~20 s 8例（28.57%）、21~30 s 13例（46.43%）、≥30 s 5例（17.86%）;围术期造口缺血2例,造口粘膜皮肤分离1例,造口周围炎3例,造口回缩0例。两组之间的排便前排便信号出现时间的增加有显著性差异,P〈0.05;功能锻炼组造口并发症明显比对照组减少。结论腹膜外结肠造口手术后功能锻炼使患者排便信号明显提前出现,而且增强了排便信号,使造口并发症减少,明显改善了患者生活质量。
Objective To evaluate the effect of functional exercise on patients who receive extraperitoneal sigmoidostomy for rectal cancer,and explore ways to improve life quality of patients with rectal cancer after operation.Methods Fifty-one patients with rectal cancer who received operations in our hospital were divided into the simple extraperitoneal sigmoidostomy operation group（the control group,n=23） and the functional exercise after extraperitoneal sigmoidostomy operation group（functional exercise group,n=28）.Signal before defecation and complications were analyzed and compared between the two groups.Results The ratio at different time of emerge of signal before defecation were 8.69%,43.48%,30.44%,4.34% in 5-10 s,11-20 s,21-30 s and above 30 s in the control group and were 7.14%,28.57%,46.43%,17.86% in 5-10 s,11-20 s,21-30 s and above 30 s in the functional exercise group,respectively.There were three patients of sigmoidostomic ischemia,two of dermomucosa isolation,eight of inflammation around the sigmoidostomy and two of sigmoidostomic decurtation in the control group and two of sigmoidostomic ischemia,one of dermomucosa isolation,and three of inflammation around the sigmoidostomy in the functional exercise group.The time of defecation signal in both groups increases significantly（P〈0.05）.The complications were fewer in the functional exercise group than in the control group.Conclusion Functional exercise after extraperitoneal sigmoidostomy can make defecation signals appear earlier,intensify defecation signals,reduce complications and improve life qualify of patients
