甲状腺微小癌高频彩色多普勒超声诊断分析 被引量:7

摘要 目的探讨甲状腺微小癌(TMC)的彩超声像图特点。方法回顾性分析38例TMC患者的临床资料作为观察组,全部病例术前均行超声检查,且均经手术及病理证实;另选同期35例具有相似超声声像图特征的甲状腺良性病变患者作为对照组。结果 38例TMC患者二维超声微小结节检出率为100%,声像图表现主要有甲状腺微小癌多为较低回声区,内部回声欠均匀,后方回声可见轻度的增强,13例结节内可见有微小钙化灶,大多无明显的血流信号,6例出现同侧淋巴结转移。观察组与对照组低回声、边界清楚、边缘光滑、微钙化灶的比例差异显著。结论高频彩超可根据肿块性质、形态和血供信息以鉴别肿块良恶性,对结节的定位、数目和大小的估计明显优于其他影像学检查,高频彩色多普勒超声是诊断TMC的首选检查方法。 Objective To investigate the imaging characteristics of color Doppler for thyroid gland microcarcinoma(TMC).Methods The clinical data of 38 patients who had thyroid gland TMC were reviewed.All of the patients received high frequency color Doppler examinations before surgery,and confirmed by surgery and pathology.Another 35 patients at the same period with benign thyroid disease who had similar ultrasonic imaging characteristics were selected as control group.Results Two-dimension ultrasound detection rate in 38 cases of TMC with small nodules was 100%.TMC mostly showed low heterogeneous echo area and posterior echo showed mild enhancement.There were 13 cases of nodular micro-calcifications which showed no obvious blood flow,and 6 cases of ipsilateral lymph node metastasis.The rates of low echo,clear borders,smooth edges and micro-calcifications showed significant differences.Conclusion High-frequency color Doppler ultrasound can identify benign or malignant carcinoma,according to the mass nature,morphology and blood,estimated position,number and size of nodule,which is better than other imaging techniques.High frequency color Doppler ultrasound is the first choice for TMC diagnosis.
