摘要
目的观察腹腔镜上消化道溃疡穿孔修补术的近期疗效。方法回顾性分析2009年3月~2012年4月,本院接受上消化道溃疡穿孔修补术的96例患者的临床资料,其中腹腔镜手术组46例,开腹手术组50例,比较2组患者手术情况、术后恢复情况、术后并发症及费用。结果腹腔镜组术中出血量显著少于开腹组,术后疼痛评分显著低于开腹组,下床活动时间、排气时间、进食时间及住院天数显著短于开腹组;腹腔镜组切口并发症发生率显著低于开腹组;腹腔镜组虽然手术费显著高于开腹组,但其他费用均显著低于开腹组。结论腹腔镜上消化道溃疡穿孔修补术疗效确切,且与开腹手术相比优势明显,值得推广。
Objective To observe the short-term efficacy of the laparoscopic patch for upper gastrointestinal ulcer with perforation.Methods The clinical data of 96 patients who received laparoscopic patch for upper gastrointestinal ulcer with perforation from March 2009 to April 2012 were retrospectively analyzed.The patients were divided into laparoscopic surgery group（n=46） and open surgery group（n=50）.The surgery condition,recovery condition and complications after surgery and cost were observed and compared.Results The bleeding amount in laparoscopic group was smaller than that in open surgery group,and the pain score was lower than that in open surgery group.The off-bed time,flatulence time,food-taking time and length of hospital stay in laparoscopic group were obviously shorter than those in open surgery group.The incidence of incision complications in laparoscopic group was obviously lower than that in open surgery group.Although the operation cost in laparoscopic group was obviously higher than that in open surgery group,the other costs were lower.Conclusion The laparoscopic patch for upper gastrointestinal ulcer with perforation is effective,and has obvious advantages compared with open surgery.Thus it is worth promoting.
