振幅整合脑电图对昏迷患者近期预后的预测价值 被引量:7

摘要 目的初步探讨振幅整合脑电图对昏迷患者近期预后的判断价值。方法选择神经重症监护病房(NICU)住院的昏迷患者52例,昏迷程度分为浅昏迷、中昏迷、深昏迷三个水平,发病3d内床旁描记振幅整合脑电图(aEEG),aEEG分为正常aEEG、轻度异常aEEG、重度异常aEEG三类。同时按格拉斯哥-匹兹堡昏迷量表(GPCS)进行评分,分析aEEG分级、GPCS评分在昏迷患者近期预后预测中的价值。结果 aEEG分级的最佳截断点为正常aEEG,GPCS评分的最佳截断点为21~27分;aEEG预测死亡的敏感性、特异性、总正确率分别为79.3%、87.0%、69.2%,GPCS预测死亡的敏感性、特异性、总正确率分别为62.1%、73.9%、69.2%。结论 aEEG可作为客观评估昏迷患者近期预后的指标之一,aEEG预测死亡的准确率高于GPCS评分。
