摘要
采用滑膜切除术加缝匠肌肌蒂骨块植入治疗儿童股骨头缺血性坏死38例,经1年以上随访,结果优32例,占84.2％;良5例,占13.2％;优良率达97.4％。该术适宜于Ⅰ期晚期、Ⅱ期、Ⅲ期、Ⅳ期早期患者,年龄越小,效果越好,其关键在于促进侧枝循环的建立和降低关节内高压。
328 children with Perthes' disease were treated by synovectomy and implantation of borne mass with a sartorius pedicle in the past 5 years. After operation was applied skin traction,and then hip spica for about 3 months.The followup of more than 12 months showed that excellent and good results were obtained respectively in 32 and 5 cases with 84.2% and 13.2%,the total of which was 97.4%.The procedure is suitable for the patients at the lateⅠ,or Ⅱ or early Ⅲ stage.The younger the patients,the better the results.The key to the treatment lies in the rehabilitation of the collateral circulation and the reduction of the intraarticular pressure.
