摘要
目的:探讨和分析应用腹腔镜下胆道镜保胆取石术治疗胆囊结石的临床疗效,总结其临床价值。方法:回顾性分析具有完整临床资料的2005年10月至2010年10月来我院就诊的胆囊结石患者123例,观察组患者采用腹腔镜辅助下胆道镜保胆取石术,切除胆囊底部,胆道镜检查取石后缝合胆囊。观察治疗胆囊结石的临床疗效。结果:123例患者经手术治疗后均顺利出院,治疗时手术时间为70-l00min,平均手术时间为90min。术中出血量平均为13.25mL,肛门的排气时间平均为27h。术后未出现出血、胆瘘、胰腺炎、腹腔积液等并发症。术后随访6个月至1年,未发现结石复发。结论:腹腔镜下胆道镜保胆取石术治疗胆囊结石是胆囊结石安全、有效、微创,临床治疗效果显著,不良反应和并发症少,能提高患者的术后的生活质量,值得临床借鉴和推广。
Objective： To discuss and analyze the application of laparoscopic choledochoscopic gallbladdersparing operation in treatment of cholelithiasis clinical efficacy and summarize its clinical value.Method： A retrospective analysis of a complete clinical data from October 2005 to October 2010,123 cases of gallstone patients in our hospital.The observation group were treated by endoscopic cholecystolithotomy assisted by laparoscopy,resection of the gallbladder bottoms and cholangioscopy stones after suture gallbladders.The clinical efficacy of the treatment of gallbladder stones was observed.Result： 123 patients treated by operation were successfully discharged.The operation time was 70-l00 minutes and the average operation time was 90 minutes.The amount of bleeding in operation was 13.25ml in average and anal exhaust time was 27 hours in average.There was no postoperative complication such as bleeding,biliary fistula,pancreatitis,abdominal effusion,etc.After a follow-up of 6 months to 1year,no stone recurrence was found.Conclusion： Laparoscopic choledochoscopic gallbladdersparing operations for the treatment of gallbladder stones are safe,effective,and minimally invasive.They have remarkable clinical therapeutic effects with a few adverse reactions and complications.They can improve the patients＇ life quality after operation,worthy of the clinical reference and popularization.
