摘要 目的 为探讨体表心电图 P波离散度预测特发性阵发型心房纤颤 (PAF)发生的可行性 ,寻找无创性筛选特发性PAF患者与健康人群的一种简单有效方法 .方法 对同期年龄和性别相当的 40名特发性 PAF患者和 40名健康成年人(对照组 )窦律时体表心电图的 P波离散度进行测量分析比较 .结果 特发性 PAF患者 P波离散度明显高于对照组(6 0 .75± 8.31vs42 .10± 7.13,P<0 .0 5 ) ,P波离散度为 5 0ms时筛选特发性 PAF患者的灵敏度和特异度分别为 95 .0 %和 6 3.2 % ,阳性预告值和阴性预告值分别为 73.1%和92 .3% .结论 体表心电图 P波离散度可作为无创性预测特发性 AIM To observe the feasibility of P-wave dispersion of surface electrocardiogram (ECG) in predicting idiopathic paroxysmal atrial fibrillation (PAF) and find a simple effective method to noninvasively separate patients with PAF from healthy subjects. METHODS We measured and compared P-wave dispersion in 40 patients with idiopathic PAF with 40 age-matched healthy control subjects from the surface ECG under sinus rhythm. RESULTS P-wave dispersion was found to be significantly higher in patients with idiopathic PAF than in healthy subjects (60.75±8.31 vs 42.10 ± 7.13 , P <0.05). P-wave dispersion valve of 50 ms separated patients from control subjects with a sensitivity of 95% and a specificity of 63.2%, and with a positive predictive value of 73.1% and a negative predicative valve of 92.3% . CONCLUSION This suggests P-wave dispersion is a simple effective electrocardiographic marker that could be used for noninvasive prediction of idiopathic PAF.
