联合手术摘出睫状体部非磁性异物的探讨

摘要 目的探讨显微联合手术摘出睫状体部非磁性异物的手术方法及效果。方法行显微玻璃体切除术联合超声乳化术摘出术睫状体部非磁性异物38例(38眼)。结果38眼均为术中一次摘出成功,经角膜缘切口摘出者25眼,经巩膜切口摘出者13眼。术后视力2眼光感~手动,2眼指数,11眼0.02~0.08,19眼0.1~0.3,4眼优于0.3,优于术前视力,差异有统计学意义(X^2=13.277,P〈0.05)。术后无严重并发症发生。术前19眼合并视网膜脱离。随访4—43个月有17眼(89.5%)视网膜保持复位。结论采用显微联合手术能顺利摘出睫状体部的非磁性异物,降低了眼内组织的损害,使患者最大限度地恢复视功能。 Objective To investigate the surgical technique and its effects of extraction of nonmag- netic foreign body from the ciliary region. Methods A clinical retrospective study of 38 consecutive patients (38 eyes) treated for ocular penetrating trauma was conducted since March of 2006 to June of 2009. All the patients were done with combined microsurgery in extraction of nonmagnetic foreign body from the ciliary- body. Results All the nonmagnetic foreign bodies from ciliary of 38 eyes were successfully extracted at once attempt, the achivement ratio was 100%. 25 cases were extracted via limbus, while 13 cases via sclera incision. Postoperative visual acuity were : 4 eye above 0.3, 19 eyes 0.1 - 0.3, 11 eyes 0.02 - 0.08, 2 eyes finger counting, 2 eyes hand movement. There was significant difference between pre and post operative visual acuity (X^2 = 13. 277, P 〈 0.05). No severe complication was oceured and 17/19 (89.5%) eyes got reattaehment during the follow up. Conclusion In this nonrandomized study, the combined microsurgery can extract the foreign body from ciliary body successfully. The wounded eyes could be treated without de-lay, and the good visual results could be obtained in eyes almost undergoing prompt foreign body removal, especially using the technique of combined microsurgery.
