摘要
目的探测高龄股骨颈骨折后卧床期间足踝伸屈运动频率与呼吸同步的运动方式对下肢静脉血流动力学的影响,评价该运动方式对预防下肢深静脉血栓效果,以便于患者掌握方法,为健康宣教提供理论依据。方法用彩色超声多普勒检测10名高龄股骨颈骨折患者健侧股静脉在各种呼吸状态下的血流动力学指标。结果深呼吸、足踝伸20°屈45°、深呼吸+足踝伸20°屈45°同步进行三种运动方式均可增加股静脉血流速度;后两者差异显著。结论深呼吸与足踝伸20°屈45°同步进行运动方式对股静脉血流的促进作用最强,指导高龄股骨颈骨折后患者卧床期间进行该种方法运动,操作方法简单,易掌握。为预防高龄股骨颈骨折后卧床期间下肢静脉血栓的发生,指导患者做深呼吸与足踝伸20°屈45°同步进行运动方式最为理想。
Objective To investigate the angle of foot dorsal flexion movement and duration of right lower extremity venous hemodynamics, in order to guide patients and provide a theoretical basis for effective activities, so as to prevent deep vein thrombosis. Methods Color Doppler ultrasound detection of 10 healthy volunteers subjects with bilateral femoral vein of adult foot and ankle flexion and extension angles at the ankle flexion and extension movement with the different duration of the state of the stock index changes in hemodynamics. Results The ankle's movement of extension 20°-flexion 45°is significant different with that of ankle - flexion 20°, the former can significantly increase the peak speed and average speed of the femoral vein blood flow; the statistical difference can be observed between the results of static state and that after the movement of ankle extension 20 °flexion 45 oin 2 seconds but no statistical difference between the movement of ankle extension 20°flexion 45°in 2seconds and that in 3 seconds; there is statistical significance between 4 seconds' ankle extension 20° flexion 45°movement and 5 seconds' of that. Conclusion Ankle's alternatively movement of flexion 45° and extension 20° in the range of 2 seconds to 3 seconds can promote the flow of the right femoral vein to the largest extend, which can be adopted as an ideal guide to the patients' preventive exercises of ankle joint.
