摘要
目的探讨超声吸引刀(CUSA)在肝包虫全囊切除中分离组织的效果及技巧。方法回顾性分析2007年7月至2010年3月使用超声吸引刀行肝包虫全囊切除组(38例)与常规肝包虫全囊切除组(42例)在手术时间、术中出血量、手术后3d引流量、及住院时间方面进行对比。结果超声吸引刀组的平均手术时间(75.0±19.2)min,常规手术组为(96.0±22.8)min,差异有统计学意义(P<0.05),术中均无大血管损伤;超声刀组术中出血量为(120.0±13.4)g,常规手术方式术中出血量为(210.0±33.5)g,明显少于常规手术组(P<0.01),超声刀组术后3d腹腔引流量为(110.0±45.8)ml,明显少于常规手术组(325.0±105.4)m(lP<0.01),超声吸引刀组住院时间(7.2±3.5)d,明显少于常规手术组(11.5+3.3)d(P<0.01)使用超声吸引刀行肝包虫全囊切除术组在手术时间、术中出血量、术后3d引流量及住院时间方面均少于常规肝包虫全囊切除术组。结论:在肝包虫全囊切除术中使用超声吸引刀进行分离是安全、有效的,值得在临床推广。
Objective To discuss the effect of surgical treatment of 80 hepatic echinococcosis patients by pericystectomy aided with CUSA.Methods The 80 hepatic echinocococcosis cases were divided into complete CUSA cyctectomy group consisted of 38 cases and conventional cystectomy group consisted of 42 cases.The results in both groups were compared and analyzed.Results The average operation time in CUSA group and conventional group were（75.0±19.2）min and（96.0±22.8）min,,showing significant differences（P0.05）：the volume of blood loss and 3 day drainage were.（120.0±13.4）g and（110.0±45.8）ml in CUSA group and（210.0±33.5）g and（325.0±105.4）ml in conventional group（P0.01）;the hospitalization days of CUSA（7.2±3.5 days）was also significantly shorter than that of the conventional group（11.5＋3.3 days）,showing significant differences（P0.01）.Conclusion Complete cyctectomy aided by CUSA is an effective and reliable method for treatment hepatic echinococcosis patients.
