摘要
目的探讨成人肱骨小头移位性骨折应用Herbert螺钉治疗的方法与疗效。方法采用Herbert螺钉治疗成人肱骨小头移位性骨折7例。按照Bryan-Morrey分型,Ⅰ型4例,Ⅲ型1例,Ⅳ型2例。术后行肘关节功能评定。结果 7例术后均获随访,时间12～48个月,平均30个月。术后切口均一期愈合,无血管神经损伤及骨块缺血坏死。根据Mayo肘关节功能评分(MEPS)行肘关节功能评定,优5例,良2例。结论 Herbert螺钉治疗Bryan-Morrey分型中的Ⅰ、Ⅲ、Ⅳ型肱骨小头骨折,固定可靠,操作简单,术后可早期进行锻炼,肘关节功能恢复好,是成人肱骨小头移位性骨折的理想固定方法。
Objective To study the surgical methods and efficacy of Herbert screws in treatment of the displaced fractures of adult’ capitulum humeri.Methods 7 patients with displaced fractures of capitulum humeri were treated with Herbert screws,including 5 males and 2 females and aging 18 to 55 years with an average age of 32 years.All patients were followed up for 12 to 48 months（30 months on average）.According to Bryan-Morrey’s criterion for the displaced fractures of capitulum humeri,4 cases were classified as typeⅠ,1 case as type Ⅲ,2 cases as type Ⅳ.The outcome was evaluated by Mayo Elbow Performance Score（MEPS）.Results All the incisions were healed by first intention and there were no vascular and nerve injury complications.There was completed fusion of fracture and not ischemia necrosis of bone.According to Mayo Elbow Performance Score（MEPS） for the function of elbow joint,there were 5 cases of excellent,2 of good.Conclusion The treatment of displaced fractures of adult’ capitulum humeri with Herbert screws has the advantages of reliable fixation and easy operation,early postoperative functional exercise could be executed by patients and the recovery of eblow joint would be well done.Herbert screws fixation is an ideal surgical method for treatment of displaced fractures of adults’ capitulum humeri.
