摘要
目的观察纤溶酶联合氯吡格雷与拜阿司匹林治疗进展型脑梗死的临床疗效。方法选取武钢第二职工医院2009年3月至2011年2月确诊的86例进展型脑梗死患者,随机分为治疗组和对照组,治疗组给予氯吡格雷与拜阿司匹林联合纤溶酶治疗,对照组给予氯吡格雷与拜阿司匹林治疗,两组疗程均为14d,比较两组患者的神经功能缺损程度评分、临床疗效、血液流变学指标以及毒副作用和安全性。结果两组治疗后神经功能缺损程度评分均改善,但治疗组(13.98±1.02)明显优于对照组(17.11±0.96),差异有统计学意义(P<0.05);治疗组显效率及总有效率分别为72.50%、92.5%,明显高于对照组的56.52%、81.8%,差异均有统计学意义(P<0.05);治疗后两组患者高切粘度、低切粘度、血浆粘度和纤维蛋白原含量均降低(P<0.05),治疗组明显优于对照组。两组患者间毒副作用和安全性比较差异无统计学意义(P>0.05)。结论纤溶酶联合氯吡格雷与拜阿司匹林治疗进展型脑梗死近期疗效显著,可以作为临床上治疗进展型脑梗死患者的一种有效方法。
Objective To study the clinical efficacy of plasminogen combinded with clopidogrel and Aspirin on the treatment of progressive cerebral infarction. Methods 86 confirmed progressive cerebral infarction cases in our hospital were collected from March 2009 to 2011 February, and randomly divided into the treatment group and control group. The treatment group patients were treated with Clopid6grel, Aspirin and Plasminogen while the control group was given the Clopidogrel and Aspirin for 14 days. Then the neurologic impairment score, clinical efficacy, hemorheology, side effects and security were compared between the patients in two groups. Results Compared with the control group, the treatment group patients showed a significant improvement on the extent of neurolog!c impairment score, clinical efficacy and the hemorheological parameters （P〈0.05）, while there were no significant difference on the side effects and safety between the two groups （P〉O.05）. And the high shear viscosity, low shear viscosity, plasma viscosity and fibrinogen levels were lower in the treatment group （P 〈0.05）, the significantly difference were observed between the two groups. Conclusions Clopidogrel and Aspirin combinded with Plasminogen treatment on the progressive cerebral infarction is effective and safe in the acute phase, and there are no obvious side effects. Plasminogen can be used as an effective method for the clinical treatment of patients with progressing stroke.
