摘要
目的:探讨临床药师基于药品不良事件开展临床药学服务的方式。方法:通过典型案例分析,介绍临床药师从不同的角度识别药物不良事件,协助医生制订合理、有效的治疗方案。结果:临床药师可通过采集用药史、关注药物相互作用、监测血药浓度及检索国内外文献等方面鉴别药物不良事件。结论:临床药师应积极参与到临床医疗实践中,提高专业水平,及时为临床医疗提供科学有效的参考依据,使临床药师在临床合理治疗,提高疗效,保障安全和促进医患和谐的实践中发挥作用和体现价值。
AIM： To discuss clinical practice of clinical pharmacists participating in adverse drug events （ADEs） identification. METHODS： Based on typical case analysis, clinical pharmacists identified ADEs from different points of view and help physicians work out more reasonable and effective pharmaco-therapeutic programs. RESULTS： Clinical pharmacists should be able to indentify ADEs by ways of medication history collecting, drug-drug interaction appreci- ation, therapeutic drug monitoring and literature retrieval. CONCLUSTION： Clinical pharmacistscan gradually integrate into the medical care team and play the important roles in it by partic- ipating in clinical medication therapy. Clinical pharmacists should also make effect to evaluate clinical drug therapeutics to ensure drug safety of patients and establish harmonious relations between doctors and patients.
