摘要
目的评价少数民族农村社区重性精神障碍综合防治模式的效果。方法调查项目实践区重性精神障碍患者的检出率和防治康复措施的覆盖率;在项目试验区采用单纯随机抽样方法抽取实施社区防治康复管理的200例精神分裂症患者作为研究组;在未实施防治康复管理社区采用单纯随机抽样方法抽取200例精神分裂症患者作为对照组,评价社区精神疾病防治康复措施对重性精神疾病患者治疗依从性、病情、社会功能和复发的影响,以及对患者家庭经济负担的影响。结果重性精神障碍患者的检出率和防治康复措施的覆盖率逐年提高。研究组的规则用药率、显进率和社会参与率逐年提高,差异均有统计学意义(P<0.01),并高于对照组同期水平,差异有统计学意义(P<0.01);而复发率低于对照组同期水平,差异有统计学意义(P<0.05);实施社区精神障碍防治康复措施,可使患者家庭每年减少治疗费用1 146元/人次。结论少数民族农村社区重性精神障碍综合防治模式的实施,可以提高重性精神障碍患者的检出率和防治康复措施的覆盖率。提高患者的药物依从性,控制患者的病情,恢复患者的社会功能,防止其复发,减轻患者家庭经济负担。
Objective To assess the effect of comprehensive prevention and control on serious mental disorders in minority rural communities in Guangxi.Methods The detectable rate of serious mental disorders and the coverage rate of prevention and control in the project conducted areas were investigated.The simple randomly smpling was taken to choose 200 patients with schizophrenia who had been given the community prevention and control management in the project pilot area,and 200 patients with schizophrenia who had not been given the management as controls.The influence of the management to the therapy compliance,disease condition,social function and recurrence of disease,as well as financial burden on families of patients with serious mental disorders.Results The detectable rate of serious mental disorders and the coverage rate of prevention and control had gradually increased year by year.The rates of regulated medication,evidently improvement and social participation in the trial group had gradually increased（P〈0.01）,and were significantly higher than those in the control group at the same period（P0.01）;and the rate of recurrence was lower than that in the control group at the same level（P〈0.05）.The implementation of the community prevention and control measures could reduce RMB 1 146 yuan cost per patient family every year.Conclusion The implementation of comprehensive prevention and control measures on serious mental disorders in minority rural communities can improve the detectable rate of serious mental disorders and the coverage rate of prevention and control.It can enhance the patients′ medication compliance,control the disease condition,restore the social function of patients,prevent the recurrence of disease,and reduce the financial burden of patient′s family.
