胃癌组织中p16蛋白和Ki-67抗原的表达及意义 被引量:2

摘要 目的 探讨 p16蛋白和 Ki- 6 7抗原在胃癌发生、发展中的作用。方法 应用免疫组织化学法检测 p16和 Ki- 6 7在 2 0例正常胃组织和 6 0例胃癌组织中的表达。结果 胃癌组织中 p16蛋白表达阳性率(48.9% )明显低于胃正常组织 (90 % )。 p16蛋白表达与胃癌的分化程度、淋巴结转移及浆膜和浆膜外脏器的累及有关 (P<0 .0 5 )。Ki- 6 7在胃癌组织中的标记阳性率明显高于胃正常组织 (P<0 .0 5 ) ,伴有淋巴结转移的胃癌组织Ki- 6 7标记阳性率较高。 p16蛋白表达与 Ki- 6 7分级呈明显负相关 (P<0 .0 5 ) ,p16阴性表达的细胞增生活性明显增高。结论  p16蛋白表达缺失及其引起的细胞周期调控失常在胃癌的发生、发展中起一定作用 ;p16蛋白的检测有助于估计胃癌患者的预后、指导临床治疗 ;联合检测 p16和 Ki- 6 Objective To study the clinical significance of the p16 protein and Ki 67 antigen expression in the occurence and development of gastric carcinoma.Method By using immunohistochemical methods,we detected the expression of p16 protein and Ki 67 antigen in 20 cases of normal gastric tissue and 60 cases of gastric carcinoma.Results The positive rate of p16 protein in gastric carcinoma was lower than that in normal gastric tissue.The expression of p16 was related to the degree of tumor differentiation,lymph node metastasis and the depth of invasion.The positive rate of Ki 67 LI (labelling index) in gastric carcinoma was higher than that in normal gastric tissue,and the gastric carcinoma with lymth node metastasis had a still more expression of Ki 67.A negative correlation was found between the expression of p16 protein and the Ki 67 LI;the cell proliferative activity in gastric tumors with a negative p16 expression was significantly higher than that in those with a positive p16 expression.Conclusion Deletion of p16 gene and subsequent irregulation of cell cycle play an important role in the occurrence and development of gastric carcinoma.Detection of p16 protein will contribute to the prognosis and therapy of gastric neoplasm.Both p16 and Ki 67 may help to predict the possibility of lymth node metastasis in gastric carcinomas.
