冠状动脉内置入非药物支架和药物支架对凝血及抗凝血系统的影响 被引量:3

摘要 目的分析并评价冠状动脉内支架置入对组织因子(TF)以及组织因子途径抑制物(TFPI)所产生的影响。方法 2009年4月-2011年3月收治的55例急性冠状动脉综合征患者按照就诊时间分为2组,其中30例置入非药物支架的患者作为A组,25例置入药物支架的患者作为B组。对两组患者置入支架之前、置入后即刻、置入后24及48 h后血液中的TF及TFPI含量进行测定,比较分析两组患者的TF与TFPI的浓度随时间改变情况。结果 A组共计置入43枚支架,B组共计置入40枚支架。检测结果表明,A组与B组的患者置入支架后即刻、置入后24 h和48 h血液中TF水平上升,均高于各组对应的置入支架前浓度(P均<0.05);A组患者置入支架后即刻血液中TFPI水平比置入支架前上升,置入支架后48 h下降到基线值之下(P<0.05),B组同样存在类似的变化趋势。置入支架后即刻尽管出现上升,但差异无统计学意义(P>0.05)。两组之间在上述对应的各时间点的TF与TFPI水平差异无统计学意义(P>0.05)。结论目前在药物治疗的前提下,冠状动脉内药物支架与非药物支架置入对凝血及抗凝血系统具有类似的影响,并不能加快系统内高凝状态的产生。
