摘要
目的对射频消融(RFA)联合肝动脉化疗栓塞(TACE)治疗与单纯射频消融治疗对小肝癌(≤3cm)的疗效进行比较。方法回顾分析我院2007年1月～2012年2月进行的72例小肝癌射频消融治疗患者的临床资料。72例中39例灶行TACE联合RFA,33例单纯行RFA。比较两组肿瘤完全消融率,复发率,复发时间,术后1、2、3年生存率等情况。结果 TACE联合RFA组肿瘤完全消融率92.31%,局部复发率28.21%,局部复发时间(13.75±3.67)月,1、2、3年的生存率分别为89.74%、76.92%和66.67%。单纯RFA组肿瘤完全消融率90.91%,局部复发率30.30%,局部复发时间(12.23±2.68)月,1、2、3年的生存率90.91%、72.73%、69.70%。结论射频消融联合肝动脉化疗栓塞及单纯射频消融治疗小肝癌(≤3cm)疗效相当,因此联合治疗对小肝癌可能没有必要。
Objective To compare the curative effect of transcatheter arterial chemoembolization （TACE） combined with radiofrequency ablation （RFA） and only radiofrequency ablation for small hepa- tocellular carcinoma （≤3cm） treatment. Methods The clinical data of 72 small hepatocellular patients with radiofrequency ablation from Jan 2007 to Feb 2012 in our hospital （39 for TACE combined with RFA and 33 for only RFA） was retrospectively analyzed. The complete ablation rate,the local recurrence rate,the recurrence time,the 1-, 2-, 3-year survival rates were compared between the two groups. Results The complete ablation rate, the local recurrence rate, the recurrence time, the 1--, 2--, 3- year survival rates were 92.31 %,28.21 %, （13.75± 3.67） m, 89.74 %, 76.92 %, 66.67 % respectively in the TACE combined with RFA group,and 90.91%,30.30% ,（12.23±2.68）m,90.91 %, 72.73 %, 69. 70% respectively in the only RFA group. Conclusions transcatheter arterial chemoembolization combined with radiofrequency ablation and only radiofrequency ablation have equivalent effects in small hepatocellular carcinoma treatment. So the combined treatment for small hepatocellular carcinoma may not be necessary.
