摘要
目的探讨手术联合康复治疗对手部烧伤后瘢痕挛缩畸形的治疗效果。方法回顾性分析我院自2009年1月至2012年1月收治的手部烧伤后瘢痕挛缩畸形患者52例,患者均接受常规的临床手术治疗和护理治疗。将术后的康复治疗随机分为试验组和对照组,试验组采用专业的康复方法;对照组采用常规的门诊指导性的手部功能锻炼。两组患者分别于术后3个月测量康复评定指标手指关节总活动度和日常生活活动能力。结果试验组和对照组康复治疗前后手指关节总活动度的对比,治疗前后两组均有统计学意义(P<0.05),治疗3个月后,试验组和对照组相比,差异有统计学意义(P<0.05)。试验组和对照组康复治疗前后日常生活活动能力的对比,试验组优于对照组,差异有统计学意义(P<0.05)。结论烧伤后手部瘢痕挛缩手术结合专业康复治疗的疗效明显优于术后单纯的指导性治疗,可以明显改善手部运动的协调性和灵活性,值得临床推广。
Objective To explore the therapeutic effect of surgery joint rehabilitation for bums scar contracture deformity. Methods 52 eases of patients with cicatricial contracture received by our hospital from January 2009 to January 2012 were retrospectively analyzed,all of the patients received routine clinical treat- ment and nursing treatment. The postoperative rehabilitation was randomly divided into the experimental group and the control group, the experiment by the methods of professional rehabilitation way; the control group with normal outpatient guiding hand function exercise. Rehabilitated finger joints total mobility and dai- ly life activities after 3 months of the two groups were measured. Results The difference of finger joints total mobility between the experimental group and control group before and after rehabilitation was statistically significant （P 〈 0.05 ） , after 3 months of treatment, the difference between the experimental group and the control group was statistically significantly different （ P 〈 0.05 ）. Daily life activities of the experimental group before and after rehabilitation treatment was better than the control group, the difference was statistically sig- nificant（ P 〈 0.05 ）. Conclusion Burned hand scar contracture surgery combined with rehabilitation treatment, which can significantly improve the hand movement coordination and flexibility, has better effects than the simple guiding treatment, thus is worth of clinical promotion.
