摘要
目的分析研究血液透析对糖尿病肾病与非糖尿病肾病的临床疗效,以减少患者的并发症,提高其生活质量和长期的生存率。方法回顾2007年1月至2008年1月在我院治疗的52例糖尿病肾病患者的临床资料,选取同期收治的50例非糖尿病肾病患者的临床资料作为对照。比较两组患者的死亡原因、并发症以及生存率。结果 3年后,糖尿病肾病组患者生存28例,生存率为53.85%;对照组患者生存39例,生存率为78%,糖尿病肾病患者的3年生存率明显低于对照组,两组比较差异有统计学意义(P<0.05)。糖尿病肾病组患者因感染死亡11例,占45.83%,心血管疾病死亡5例,占20.83%,脑血管疾病死亡3例,占12.5%,其他5例,占20.83%。结论血液透析治疗糖尿病肾病疗效较非糖尿病肾病差,对于糖尿病肾病患者,尽早进行血液透析,积极控制血压、血脂、血糖,并改善患者自身的贫血状况,可以改善患者的临床症状,提高患者生存率。
Objective To explore the clinical curative effect of hemodialysis in diabetes nephrosis and nondiabetic nephropathy patients, and the measures of reducing complications, improve the life quality and long-term survival of patients. Methods A retrospective study was done from January 2007 to January 2008, 52 patients with diabetes nephrosis were selected and 50 patients without diabetes nephrosis were selected as control group. The cause of death, complications and survival of the two groups of patients were compared. Results 3 years later, there were 28 survivals（53.85% ）in the diabetic nephropathy group and there were 39 survivals（78% ）in the control group. The 3-year survival rate in the diabetes nephrosis group was lower than that in the control group, the difference was statistically significant（ P 〈 0.05 ）. In the diabetes nephrosis group, 11 case （ 45.83 % ） died of infection ; 5 cases （ 20.83 % ） died of cardiovascular disease ; 3 patients （ 12.5 % ） died of cerebrovaseular diseases ; 5 cases （ 20.83 % ） died of other reasons. Conclusion The cura- tive effect of hemodialysis in diabetic nephropathy patients is worse than that in nondiabetic nephropathy pa- tients. The diabetes nephrosis patients should begin hemodialysis as early as possible, during treatment active control of blood pressure, lipids, and sugar, and improvement of the nutritional status and anemia of the pa- tients, should be done, which can improve the clinical symptoms and the survival rate.
