摘要
目的:探讨SPECT与MRI在不同病理类型肺癌骨转移诊断中的应用价值。方法:选择我院肿瘤中心住院治疗的晚期肺癌患者150例,所有患者均接受SPECT及MRI检查,以病理学结果为标准,分析不同病理类型肺癌骨转移分布情况。比较SPECT与MRI对不同病理类型肺癌骨转移的诊断情况。结果:(1)腺癌、鳞癌及其它类型肺癌患者共发生骨转移112处、123处及34处。骨转移的部位包括脊柱、骨盆、四肢、颅骨、胸廓。(2)SPECT对腺癌骨转移检出率为86.61%,显著高于MRI的74.11%(t=5.5434,P=0.0186);SPECT对鳞癌骨转移检出率为84.55%,显著高于MRI的73.17%(t=4.7795,P=0.0288);SPECT对其它类型肺癌骨转移的检出率为85.29%,与MRI的67.65%相比无统计学差异(t=2.9423,P=0.0863)。结论:在腺癌和鳞癌骨转移的检查方面,SPECT优于MRI。
Objective：To explore the application value of SPECT and MRI in the diagnosis of different pathological types of lung cancer with bone metastasis.Methods：：150 cases of advanced lung cancer patients underwent SPECT and MRI,according to the pathology results,we analyzed the distribution of different pathological types of lung cancer bone metastases.Comparison SPECT and MRI of different pathological types of lung cancer bone metastasis.Results：（1）There were 112,123 and 34 adenocarcinoma,squamous cell carcinoma and other types of lung cancer patients with bone metastasis.Bone metastasis sites including spine、pelvis、extremities、skull and thorax.（2）The detection rate of adenocarcinoma bone metastasis using SPECT were 86.61%,significantly higher than MRI（74.11%）（t=5.5434,P=0.0186）;the detection rate of squamous cell carcinoma bone metastasis using SPECT were 84.55%,significantly higher than MRI（73.17%）（t=4.7795,P=0.0288）;the detection rate of other types of lung cancer with bone metastasis were 85.29%,compared with MRI,there were no significant difference（t=2.9423,P=0.0863）.Conclusion：SPECT is superior to that of MRI in the detection of adenocarcinoma and squamous cell lung cancer with bone metastasis.
