麦味地黄丸防治肺癌放疗致放射性肺炎40例临床观察 被引量:7

Observation on MaiWei DiHuangWan in Preventing and Treating 40 Cases of Radiation Pneumonia Induced by Radiotherapy of Lung Cancer

摘要 目的:观察麦味地黄丸防治肺癌放疗致放射性肺炎的临床疗效。方法:将78例非小细胞肺癌(NSCLC)患者分为2组,治疗组40例给予放疗联合麦味地黄丸治疗。放疗方案:放射源为6MV-X线,照射野包括原发灶+同侧肺门+纵隔。采用前后野对穿照射,照射量达到TD40 Gy/次,20次,共4周,然后缩野,改为斜野或多野照射,避开脊髓继续照射到TD60-70Gy。麦味地黄丸,8粒/次,3次/d,在放疗开始时服用,连续服用3个月。对照组单纯行放射治疗。结果:放射性肺炎发生率治疗组为15.00%,对照组为34.21%,2组比较差异有统计学意义(P<0.05)。白细胞下降发生率治疗组为27.50%,对照组为52.63%,2组比较差异有统计学意义(P<0.05)。临床影像生理评分(CRP)及身体机能状态量表(KPS)评分治疗前2组比较,差异无统计学意义(P>0.05);治疗2个月、3个月后与治疗前组内比较,差异均有统计学意义(P<0.05),且治疗组改善较对照组明显(P<0.05)。2组在治疗过程中未发现不良事件;未发现与中药治疗相关的毒副反应。结论:麦味地黄丸对肺癌放疗致放射性肺炎具有明显的防治作用。 Objective: To observe therapeutic effect ofMaiWei DiHuangWan in the prevention and treatment of radiation pneumonia induced by lung cancer. Method: All 78 patients with non-small-cell carcinoma were randomly assigned into 40 cases in treatment group and others in control group. Treatment group were given with radiotherapy and MaiWei DiHuangWan. Radiotherapy: radioactive source was line 6MV-X, radiation field contained primary tu- mor, ipsilateral hilum and mediastinum. Radiation field including anterior and posterior ones, exposure reached to Gt/times, 20 times, radiation for four weeks and then exposure field decreased to oblique-field irradiation or multi-field radiation, exposure was up to TD60-70 Gy except spinal cord. MaiWei DiHuangWan, eight pills each time, three times per day, patients began to take for three consecutive months when radiotherapy was performed. Control group received radiation only. Result: Incidence of radiation pneumonia in treatment group was 15.00%, lower than 34.21% of control group with statistical meaning (P〈0.05). Incidence of leukocyte decline in treatment group was 27.50%, lower than 52.63% of control group with remarkable difference (P〈0.05). Comparison of both groups before treating suggested no statistical meaning in CRP and KPS score (P〉0.05); comparisons within a group after treating for two months, three months and before treating demonstrated statistical meaning (P〈0.05), and observation group improved more greatly than control group (P〈0.05). No adverse reaction happened in both groups during the treatment; no toxic reaction occurred related to herbs. Conclusion: MaiWei DiHuangWan can obviously prevent and treat radiation pneu- monia induced by radiotherapy of lung cancer.
