摘要
目的通过观察2型糖尿病(T2DM)及合并冠心病(CHD)患者血清基质金属蛋白酶-9(MMP-9)、高敏C反应蛋白(hs-CRP)、白介素-6、18(IL-6、IL-18)水平变化,探讨血清MMP-9与炎性因子的相关性及其临床价值。方法选择临床确诊的182例T2DM患者,据诊断标准分为T2DM组和T2DM合并CHD(T2DM+CHD)组,酶联免疫吸附(ELISA)法检测血清MMP-9、IL-6及IL-18水平,超敏乳胶增强免疫比浊法检测血清hs-CRP水平,检测结果进行统计学分析。结果①T2DM组的血清MMP-9、hs-CRP、IL-6及IL-18水平分别显著高于对照组(P<0.01);T2DM+CHD组的血清MMP-9、hs-CRP、IL-6及IL-18水平分别显著高于T2DM组(P<0.01)。②T2DM+CHD组血清MMP-9水平分别与hs-CRP、IL-6及IL-18呈显著性正相关(r=0.672,0.591,0.603,P<0.01),相关性良好。结论 T2DM患者体内存在MMP-9与炎性因子水平的异常表达,合并CHD患者尤为显著,且血清MMP-9与炎性因子相关联。MMP-9与炎性因子参与了T2DM并发CHD的发生发展,此为早期干预提供了理论依据。
Objective To observe the levels of serum Matrix metallopeptidase 9 （MMP-9）, High-sensitive C-reactive protein （ hs-CRP）, IL-6 and IL-18, discuss the correlation between serum MMP-9 and inflammatory factor and its clinical value. Methods Total 182 cases of clinical diagnosed T2DM patients were divided into T2DM group and T2DM with CHD （T2DM ＋ CHD） group. The levels of serum MMP-9 IL-6 and IL-18 were detected with ELISA, hs-CRP was measured with particle-enhanced turbidimetric immunoassay （PETIA）. The results were analyzed using statistical methods. Results The levels of serum MMP-9, hs- CRP,IL-6 and IL-18 in T2DM group were significantly higher than those in the control group（P 〈0.01 ） ;The levels of serum MMP-9, hs-CRP, IL-6 and IL-18 in the T2DM ＋ CHD group were significantly higher than those in the T2DM group（P 〈 0.01 ）. In the T2DM ＋ CHD group, MMP-9 level was positive correlated to the levels of hs-CRP,IL-6 and IL-18（ r = 0. 672,0. 591, 0.603 ,P 〈0.01 ）. Conclusion There were abnormal expressions of MMP-9 and hs-CRP and IL-6 and IL-18 in the high-risk T2DM patients, especially the T2DM ＋ CHD patients. MMP-9 bevel was positive correlated to the levels of inflammatory factors. MMP-9 ,hs-CRP,IL-6 and IL-18 may participate in the events of CHD complicated with T2DM. This study provides a theory basis for the early intervention of T2DM patients with CHD.
