摘要
目的探讨玻璃体切割联合超声乳化白内障摘除及囊袋内植入折叠人工晶状体手术治疗增生期糖尿病性视网膜病变(DR)对角膜内皮细胞的影响。方法合适的选择增生期DR合并白内障的患者23例(23只眼),行玻璃体切割联合超声乳化白内障摘除囊袋内植入折叠型人工晶状体手术,分别于术前、术后1周、术后3个月以上不同时期采集术眼角膜内皮细胞的各项分析指标,从中分析手术对角膜内皮细胞的影响及相关因素。并同时比较单纯玻璃体切割术29例(29只眼)及单纯超声乳化白内障手术25例(25只眼)术后角膜内皮细胞的改变,从中评价糖尿病患者角膜内皮细胞的功能及手术安全性。结果组内比较单纯玻璃体切割手术组手术前后各期角膜内皮细胞密度差异不明显,超声乳化组及联合手术组手术前后有显著性差异;各组间比较超声乳化组及联合手术组无显著差异,而两者与单纯玻璃体切割组比较差异明显。结论选择性的采用玻璃体切割联合超声乳化白内障摘除及人工晶状体植入治疗增生期DR伴有白内障的手术是有效的,对于角膜内皮细胞有一定的影响,但是联合手术与单纯超声乳化白内障手术比较并无显著性差异,这种手术方式是安全的。
Objective To investigate the effet of vitreetomy combined with phacoemulsification and foldable in-traocular lens implantation in the capsular bag surgery on the corneal endothelial cells in proliferative diabetic retinopathy.Methods Select the appropriate proliferative diabetic retlnopathy and cataract were diagnosed with vitrectomy combined with phacoemulsification cataract surgery and capsular bag implantation of foldable intraocular lens in 23 cases,and before surgery,after 1 week,more than 3 months after surgery collected in different periods of the analysis of corneal endothelial cells in target,To analyze the effet of surgery on the corneal endothelial cells and related factors.And to compare the line of vitrectomy combined with phacocmulsification cataract surgery in corneal endothelial cell changes.The changes after sim-ple vitrectomy(29 cases)and simple ultrasonic cataract surgery(25 cases)in corneal endothelial cell,To evaluate the corneal endothelial cell function and operation security of diabetic patients.Results The results showed that simple vitrec-tomy group before and after surgery on corneal endothelial cell density was no significant difference,Phacoemulsification surgery group and the combined surgery group there were significant differences,Among the three groups,Phaccemulsifi-cation surgery group and the combined surgery group there were no significant differences,but the two groups there were sig-nificant differences with simple vitrectomy group.Conclusion Selective use of vitrectomy combined with phacoemulsifica-tion and intraocular lens implantation in diabetic retinopathy associated with cataract surgery is effective,It has certain influ-ence on the corned endothelial cellss But the combined surgery compared with cataract surgery alone has no significant difference,This surgical mode is safe.
