摘要
目的:探讨cyclin D1蛋白表达在判断头颈鳞癌患者预后方面的意义。方法 :选取头颈鳞癌患者96例,取肿瘤标本制作组织芯片,应用免疫组化方法检测cyclin D1蛋白的表达情况。蛋白表达与临床病理学参数的关系应用非参数检验(Kruskal-Wallis检验),与预后的关系应用Kaplan-Meier检验和Cox回归多因素生存分析,所有统计学分析均采用SPSS17.0软件包完成。结果:患者平均随访时间为6.2a。Kaplan-Meier生存分析表明,cyclin D1低表达与高表达患者相比预后更好(P=0.009);多因素生存分析表明,cyclin D1蛋白表达是唯一与总生存率密切相关的因子(HR:2.260,95%CI:1.074-4.758;P=0.032)。该蛋白表达与病理分级、临床分期、性别及发病部位等临床病理学参数无关。结论:cyclin D1表达是预测头颈鳞癌患者预后的独立因子,能够作为头颈鳞癌潜在的分子诊断和靶向治疗靶点。
PURPOSE: To explore the prognostic value of cyclin D1 expression in head and neck squamous cell carcinoma(HNSCC).METHODS: Ninety six patients with HNSCC who were treated at the Department of Oral and Maxillofacial Surgery of Ninth people’s Hospital,were enrolled in the study.Tissue microarrays were made using tumor specimen.The expression of cyclin D1 was detected by immunohistochemistry.And the association between cyclin D1 expression and clinicopathologic features was analyzed using Kruskal-Wallis test.Survival curves were calculated using the Kaplan-Meier method.Cox proportional hazard models were utilized for multivariate analyses of cyclin D1 expression and other baseline factors with OS.All calculations and analyses were performed using the SPSS 17.0 software package.RESULTS: The mean follow-up period was 6.2 years.The patients whose tumors showed a lower cyclin D1 expression had a significantly better OS than those whose tumors showed a higher cyclin D1 expression(P=0.009) by Kaplan-Meier method.A multivariate survival analysis also showed that cyclin D1 expression was the sole factor associated with prognosis(HR: 2.260,95% CI:1.074-4.758;P=0.032).But the protein expression was not associated with any clinicopathologic factors,including gender,site,pathological grade and clinical stage.CONCLUSIONS: Cyclin D1 expression is an independent predictive factor for prognosis of HNSCC patients,and may serve as a potential biomarker for molecular diagnosis and targeted therapy in future.
