摘要
目的探讨二维超声联合彩色多普勒血流显像对膀胱肿瘤的临床诊断价值。方法采用二维超声联合彩色多普勒血流显像(CDFI)对57例膀胱肿瘤择期手术患者进行术前检查,与术后病理检查结果进行比较。结果二维超声联合CDFI探查见肿瘤68个,诊断与病理检查结果总符合率为87.7%,其中良性肿瘤诊断符合率为87.5%,恶性肿瘤诊断符合率为87.9%。CDFI检查显示,肿瘤均有彩色血流信号,肿瘤基底部或瘤体内部可检测出高阻性动脉血流频谱,阻力指数(RI)为0.47～0.79。24例良性肿瘤内可探及少量血流信号,27例恶性肿瘤内可发现丰富的血流信号,6例周边可见环状血流。结论二维超声联合CDFI能提供膀胱癌的术前大致分期,有利于指导临床治疗,为制定有效的手术方案提供依据,对术后预后的评价具有重要的意义。因此,二维超声结合CDFI可作为临床诊断膀胱肿瘤的重要方法,在临床进一步推广使用。
Objective To explore the two dimensional ultrasound and color Doppler flow imaging in bladder tumor diagnosis. Methods Two dimensional ultrasound and color Doppler flow imaging （CDFI） was used in our hospital, 57 cases of bladder tumor patients undergoing selective operation for examination before operation, and postoperative pathological examination results were compared. Results Two-dimensional ultrasotmd combined with CDFI exploration see 68 tumors, diagnosis and pathological examination results of total coincidence rate is 87.7%, one benign tumor diagnosis coincidence rate is 87.5%, malignant tumor diagnosis coincidence rate is 87.9%. CDFI examination showed, the tumor had color flow signal, tumor basal or the human body can be detected with high resistance artery, resistance index （RI） 0.47-0.79. In 24 cases of benign tumor in the probe and a small amount of blood flow signal, 27 cases of malignant tumor can be found abundant blood flow signal, 6 cases of peripheral visible annular flow. Conclusion Two-dimensional ultrasound combined with CDFI can provide preoperative staging of bladder cancer roughly, is helpful for guiding clinical treatment, for the development of effective operation plan provide a basis, on postoperative prognosis evaluation has important significance. Therefore, two-dimensional ultrasonography combined with CDFI can be used as a clinical diagnosis of bladder tumor is an important method in clinical use, further promotion.
