小切口非超乳手术中硬核白内障的劈核技术 被引量:7

摘要 目的评价劈核钩联合显微镊劈核技术在硬核白内障手术中的效果。方法将在我院接受扫盲手术的硬核自内障患者随机分成两组。A组96例(96眼),应用劈核钩联合显微镊劈核技术进行白内障手术。B组89例(89眼),应用超声乳化技术进行白内障手术,两组均植入硬性人工晶状体。术前、术后1d、1个月、3个月,检查视力,术后1周、1个月、3个月检查散光度,术前、术后1个月检查角膜内皮细胞计数等,进行对比。结果A组术后1d的视力〉10.5为42例(43.7%),B组视力≥0.5者49例(55%)。两组差别有统计学意义(P〈0.05)。术后3个月A组视力≥O.5者为88例(91.6%)B组为83例(93.2%),两组差别无统计学意义(P〉0.05)。术后1周,1个月,3个月平均散光度A组为(1.41±0.92)D,(1.214-0.83)D,(0.96±0.61)D,B组为(1.37±0.81)D(1.10±0.74)D,(0.93±0,57)D。两组对比差异无统计学意义(P〉0.05)。术后A组内皮丢失率为13.1±3.6%,B组为12.45±2.9%,两组差异无统计学意义(P〉0.05)。两组均无角膜内皮失代偿等严重并发症。结论劈核钩联合显微镊劈核技术是一种方法简便、效果确切的手术方法,特别适合于基层医院硬核白内障手术。 Objective To evaluate the clinical efficacy of chop hook combine with micro-forceps nuclear chopping technique for hard nuclear cataract patients. Methods 185 hard nuclear cataract patients had been divided into two groups randomly . 96 cases (96 eyes) in group A had undergone chop hook com- bine with micro-forceps nuclear chopping technique for cataract surgery and intraocular lens implantation. 89 cases (89 eyes) in group B were performed phacoemulsification and intraocular lens implantation. Visual a- cuity, astigmatism and endothelial cells were observed before and after operation. Results The visual acui- ty was more than 0.5 in 42 cases(43.7% ) in group A and 49 cases(55% ) in group B one day after opera- tion , there was significant difference between two groups ( P 〈 0. 05 ). However the visual acuity was more than 0.5 in 88 cases(91.6% ) in group A and 83 cases(93.2% ) in group B three months after operation, there was no significant difference between the two groups (P 〉 0. 05). The astigmatism was (1.41 ± 0.92) D,(1.21 ±0.83)D,(0.96 ±0.61)D in group A and (1.37 ±0.71)D, (1.1 ±.74)D,(0.93±0.57)D in group B 1 w, 1 m,3m after operation. There were no significant difference between the two groups. The en- dothelial cell loss rate was (13.1 ±3.6)% in group A and (12.5 ±2.9)% in group B. There were no sig- nificant difference between the two groups ( P 〉 0.05 ). Conclusion Chop hook combine with micro-for- ceps nuclear chopping technique is a simple and effective way for hard nuclear cataract surgery.
