飞秒激光辅助准分子激光原位角膜磨镶术后角膜内皮细胞密度的临床研究 被引量:10

摘要 目的探讨飞秒激光制瓣对准分子激光原位角膜磨镶术(LASIK)后角膜内皮细胞计数的影响。方法入选条件为自愿接受LASIK且经常规术前检查确认为适应证的患者,共284例,年龄18~45岁。根据术中角膜瓣制作方式不同分为S组(OUP板层角膜刀制瓣)和F组(飞秒激光制瓣)。分别在术前,术后1d、1周、1个月、3个月、6个月对术眼进行角膜内皮细胞密度检查,并记录数据分析。结果所有手术病例术中顺利,术后无并发症。内皮细胞密度:F组术后6个月同术前比较明显下降(P<0.05),余两组各时间点同术前比较差异均无统计学意义(P>0.05);术后6个月F组较S组明显下降(P<0.05),余各时间点两组间差异无统计学意义(P>0.05)。结论飞秒激光制瓣术后角膜内皮细胞密度较机械板层刀制瓣的LASIK低,其机制有待进一步研究。 Objective To evaluate the effects of femtosecond laser assisted in situ keratomileusis on corneal en- dothelial cell density(ECD). Methods Two hundred and eighty-four cases, aged 18 to 45 , were enrolled. They under- went routine preoperative examinations and accepted LASIK surgeries. Patients were separated into 2 groups: S group ( cornea flap was made with a One Use Plus mechanical microkeratome) and F group (cornea flap was made with a femto- second laser). Corneal ECD was obtained preoperatively and postoperatively. Results There was no complications in all patients during and after surgery. There was no statistical difference of ECD in group S postoperatively and preoperatively (P 〉 0.05 ). However six months after surgery ECD was significantly decreased compared with that before surgery in group F (P 〈 0.05). There was no statistical difference of ECD between these two groups within three months after sur- gery (P 〉 0.05). ECD of group F at the six months after surgery was significantly decreased compared with that of group S. Conclusions ECD was lower after LASIK whose corneal flap was produced with a femtosecond laser than that with an One Use Plus mechanical microkeratome.
