摘要
目的探讨膝关节髌下脂肪垫区滑膜病变的MRI表现及诊断。方法回顾分析13例经手术病理证实的膝关节髌下脂肪垫区滑膜病变的MRI资料,包括滑膜囊肿5例,滑膜血管瘤3例和局限型色素沉着绒毛结节性滑膜炎5例,总结其MRI表现。结果滑膜囊肿在T_1WI呈低信号,T2WI呈高信号,无强化;滑膜血管瘤在T_1WI呈低信号,T_2WI呈显著高信号,增强扫描明显强化;局限型色素沉着绒毛结节性滑膜炎在T_1WI呈等信号或稍高信号,T_2WI呈稍高信号,肿块内均可见低信号含铁血黄素。结论膝关节髌下脂肪垫区滑膜病变MRI表现具有一定的诊断特征性。
Objective To explore the MRI diagnosis of the synovial diseases in the infrapatellar fat pad of knee joint. Methods Thirteen patients with pathologically proved the synovial diseases in the infrapatellar fat pad of knee joint were retrospectively reviewed, and the MRI appearances were observed. Results Synovial cyst（3 cases） were low intense on T1WI and hyperintense on T2WI without enhancement. Synovial hemangioma （5 cases）were low intense on T1WI and obviously hyperintense on T2WI with significantly enhancement. Localized pigmented villonodular synovitis （5 cases） were isointense or slightly hyperintense on T1WI and slightly hyperintense on T2WI, and hypointensity on T2WI was found in tumor due to hemosiderin deposition. Conclusion Synovial cyst, synovial hemangioma and pigmented villonodular synovitis are common synovial diseases observed in infrapatellar fat pad of knee joint,which had certain MRI characteristics.
