摘要
目的 寻求中西医结合治疗Hp相关性胃十二指肠疾病的理想药物和方案。方法 采用前瞻性、随机双盲对照研究 ,将 166例Hp阳性的慢性胃十二指肠疾病患者分为 4组 :A组 ( 4 2例 )为洛赛克或雷尼替丁 +阿莫仙 +胃泰胶囊 ;B组( 4 2例 )为丽珠得乐 +阿莫仙 +胃泰胶囊 ;C组 ( 4 1例 )为洛赛克或雷尼替丁 +胃泰胶囊 ;D组 ( 4 1例 )为对照组 ,洛赛克 +阿莫仙 +灭滴灵。结果 ( 1) 4组的Hp根除率分别为 85 .71%、83 .3 3 %、5 1.2 2 %和 85 .3 7% ,A、B、D三组之间无显著差异 (P >0 .0 5 ) ;( 2 )A、B、C三组治疗后胃粘膜内慢性炎症程度明显减轻 (P <0 .0 1) ,而D组慢性炎症无明显改善 (P >0 .0 5 ) ;( 3 )A、B、C、D各组治疗后胃粘膜炎症活动性程度均明显减轻 (P <0 .0 1) ;( 4 )A、B、C三组中仅 1例 ( 0 .8% )出现副作用 ,而D组有 4例 ( 8.9% )出现副作用 ,二者比较有显著性差异。结论 ( 1)胃泰三联疗法与标准三联疗法对Hp的根除作用相似 ;( 2 )胃泰二、三联疗法与标准三联疗法一样可显著减轻胃粘膜的活动性炎症 ;( 3 )含胃泰的二、三联疗法可显著减轻胃粘膜的慢性炎症 ,而标准三联疗法无此作用 ;( 4 )胃泰三联疗法的副作用显著低于标准三联疗法 ,且患者的依从好。因此 ,胃泰三联疗法可作为治疗Hp相关性疾病?
Objective To search for an ideal therapeutic method to gastroduodenal disease associated with Helicobacter pylori.Method This was a prospective, randomized,double blind controlled study.166 patients with H. pylori positive gastroduodenal disease were randomly allocated to four groups:group A(n=42) taking losec or ranitidine,amoxicillin and 'Wei tai capsule';group B(n=42) taking De nol,amoxicillin and 'Wei tai capsule'; group C(n=41) taking losec or ranitidine and 'Wei tai capsule';group D(n=41,controlled group) taking losic,amoxicillin and metronidazole.Results (1)The H. pylori eradication rate in groups A,B,C and D were 85.71%, 83.33%, 51.22% and 85.37% respectively ,and there were not significant differences among groups A,B and D;(2)Compared with before treatment,the intensity of chronic inflammation after treatment significantly decreased in groups A,B and C(P<0.01).On the contrary,the intensity of chronic inflammation after treatment slightly decreased in group D(P>0.05);(3)Compared with before treatment,the intensity of activity inflammation after treatment significantly decreased in all four groups(P<0.01);(4)Only 1 patient(0.8%) occurred side effects in groups A,B and C,Whereas 4 patients (8.9%) did in groups D.Conclusions (1)There were not significant differences between the “Wei tai Capsule” triple therapy and standard triple therapy in H. pylori eradication rate;(2)“Wei tai Capsule” diad or triple therapy can significantly decrease the intensity of chronic inflammation.On the contrary,standard triple therapy does not;(3)The side effects were fewer in “Wei tai Capsule” diad or triple therapy than those in standard triple therapy.So “Wei tai Capsule” triple therapy was an ideal therapeutic method to gastroduodenal disease associated with H. pylori.
